SCOPE OF WORK
FOR
AAFES
TOILET & FAUCET REPLACEMENT
CSD PROJECT NO: M5201649
Existing Conditions:
1. The AAFES Building is located at 919 Ala Moana Boulevard, Honolulu (Ewa of Fisherman’s Wharf).
2. Normal office hours are Monday to Friday during the hours from 7:30 AM – 4:30 PM.
3. The existing toilets and lavatory faucets are problematic and not efficient.
4. There is a renovation project in progress that includes exterior repair, switch gear replacement, and cooling tower refurbishment. It is scheduled to be complete by April 30, 2012 with a shutdown period in February 18 to 26, 2012 for the electrical and mechanical work.
Scope of Work:
Provide labor, materials, and equipment to accomplish the following items:
1. The contractor, or its subcontractors, shall have at least 5 years experience with the subject plumbing fixtures in Hawaii.
2. Remove existing thirty one (31) ea toilets and thirty eight (38) ea lavatory faucets and dispose of properly.
3. Clean vents and jet drain lines as best as possible.
4. Furnish and install thirty one (31) ea new wall hung Zurn 5615 toilets with elongated bowls, seats, and matching manual flush valve.
5. Furnish and install thirty eight (38) ea new Chicago 333-E2805-665PSHCP lavatory faucets with 0.5 gpm spray outlet.
6. Equal or better substitute toilets (such as Kohler) and faucets may be submitted during the question and answer period for approval. Any approved substitute(s) will be announced via HePS as an addendum.
7. Furnish and install any miscellaneous parts in order for proper function of the new toilets and faucets.
8. Furnish and install new isolation valves with access doors in each common men and women’s restroom (10 total).
9. All work shall minimize impact to facility functions. Work shall be scheduled. Weekend work is allowed.
10. Pre-Bid Meeting – none. The common and accessible restrooms on the first to fourth floors are open to the public. The fifth floor restrooms will be scheduled to be unlocked all day on December 28, 2011 for bidding purposes.
11. All questions shall be emailed to Corey Shibata by the ‘Questions Deadline’ date that is identified on the HePS solicitation.
12. Parking for workers is available for a charge of $6/day/vehicle. Passes can be obtained at DAGS Automotive Division.
13. Any damage caused by the contractor’s work shall be repaired at no additional cost to the State.
14. Follow OSHA and HIOSH requirements.
15. Keep work area clean and safe at all times. The construction area shall be roped off and contain warning signs to prevent pedestrians from entering the work area. Any closing of walkways shall be approved and coordinated with the CSD Engineer.
16. Follow prevailing wages (refer to Special Provisions, Wages pages SP-6 to 9).
17. Insurance requirements (Refer to Special Provisions, Liability Insurance, pages SP-3 to 5.
18. Performance and Payment Bonds requirement (Refer to Special Provisions, Performance and Payment Bonds, pages SP-5 to 6.
19. All costs, including taxes, travel, mileage, and any other expenses required to perform the work shall be included.
20. Work shall be completed by May 31, 2012. Once the work is started, the contractor shall complete the work without delay or interruption.
21. Upon completion, provide a minimum one year warranty written to ‘DAGS, State of Hawaii’.
22. Contractor is responsible to verify existing job site conditions.
