In 2000 the Colorado General Assembly amended and passed legislation which produced a set of standardized procedures for evaluation, identification, and supervision of juveniles who engage in sexually inappropriate behavior. In accordance with these established procedures and the provision of a safe educational environment for all students, the school district requires students who have been adjudicated or received a deferred adjudication on a sexual offense, or who are involved in pretrial supervision charges with a sexual offense, be placed on an Individual Supervision Management Plan for purposes of support and safety in the school district. Therefore, failure of the student to abide by the terms and conditions of his/her Individual Supervision Management Plan may result in disciplinary action, up to and including suspension and/or expulsion from school and the District.
Juveniles who engage in sexually inappropriate behavior can succeed in public school settings. This plan is to build a foundation for safety within the school community and address the needs of the juvenile through informed supervision.
In addition to this Individual Supervision Management Plan, approved safety plans shall be kept on file with the building informed supervisor and a copy may be forwarded to the superintendent or his/her designee as per school district policy.

The school district Board of Education policies will be followed when conducting the Individual Supervision Management Plan meeting. These include policies related to the sharing of confidential student information and shall be used to clarify expectations regarding the student’s behavior in the school setting.
This agreement shall remain in full force and effect until modified by the Multidisciplinary Team. This Individual Supervision Management Plan is transferable as a student educational record, must be maintained by the school district, and must be implemented in any school which the student is enrolled.
I understand and agree to follow the terms and conditions of the Individual Supervision Management Plan. I understand that failure to follow the Individual Supervision Management Plan will result in disciplinary action, up to and including suspension and/or expulsion from school.
Note for pre-adjudicated students: by signing this agreement the parent(s)/guardian(s) and the student are not admitting guilt to any criminal offense or delinquent act, nor waiving any due process rights in the Criminal Juvenile Court.
