Pregnancy leave (You must give notice no later than 3 months before the expected date of birth)

Expected date of birth (documentation must be enclosed – pregnancy certificate/pregnancy record).

Pregnancy leave commences on (state the first day of absence – max 6 weeks before expected date of birth)

Remember to inform Human Resources of the actual date of birth as soon as possible.

Maternity leave (you must give notice at least 8 weeks after the day of birth)

I want to take weeks after the birth (min. 2 and max. 14 weeks).

Parental leave (you must give notice of the exact placing of the weeks no later than 8 weeks after the date of birth).

The child's father is employed by the state. The child's father is not employed by the state.

The parents must jointly decide whether they want to make use of parental leave after the 14th week – including to what extent and in what way they want to divide the parental leave between them, and how many weeks each of them wants – with and without salary.
