Liability Waiver and Medical Release: I give permission for my child, , to travel to Whitetail Ski Resort in transportation provided by Oakcrest School and to ski or snowboard at Whitetail Ski Resort on February 12, 2016. I am fully aware that these activities are inherently dangerous and may result in serious injury to, or death of, the participant. Knowing, understanding, and fully appreciating all possible risks and dangers, I hereby expressly, voluntarily, and willinglyrequest Oakcrest School to allow my child to participate in this activity. In consideration of my child’s participation in the activity, I hereby waive all claims or causes of action against Oakcrest School, its officers, directors, employees, and agents arising out of my child’s participation in the activities and hereby release, hold harmless, and discharge Oakcrest School, its officers, directors, employees, and agents from all liability in connection therewith. Furthermore, I give any Oakcrest parent, faculty, and/or staff present on the trip permission to seek out medical attention for my child in the event of an emergency.

Ability Level Descriptions:
Beginner: First time or skied/boarded a couple of times only.
Intermediate: Can ski parallel or almost parallel, with minimal wedging in turns, in some or most intermediate terrain, but not in advanced terrain. Boarders can carve on both edges and possibly do reverts. Some use of terrain park.
Advanced: Can ski in parallel form on advanced terrain. Boarders can carve in all terrain and board comfortably at higher speeds. More confident in terrain park.

Lift ticket to ALL lifts and slopes. (NO rental equipment or lessons)
Package 2 - $84(Ski or Snowboard)Ability Level
Lift ticket valid on “Beginner Terrain” ONLY, rental equipment (including helmet) andski lesson.
Package 3 - $97(Ski or Snowboard)Ability Level
Lift ticket to ALL lifts and slopes, rental equipment (including helmet) andski lesson.
(Note: The ski resort would charge $10 if you switch from ski to snowboard equipment or visa versa)
Transportation: Use school-provided buses - $25/person(The fee could change slightly depending on
final counts, but this is not likely.)

Lunch: Pre-paid lunch at the lodge ($10/person after the 40% discount)
