Application for a Standard Bus Permit for a Minibus designed to carry between 8 and 16 passengers (not including the driver)

INSTRUCTIONS

Two copies of this form are to be completed; the FIRST COPY is to be sent to the Scout Information Centre with the fee of £5.00 and the SECOND COPY is for your own vehicle records. Please complete the form in BLOCK CAPITALS. All cheques to be made payable to ‘The Scout Association’, should you wish to pay by Credit/Debit card then please see overleaf. Please note that you must comply at all times with the ROAD VEHICLES (Construction and Use) Regulations 1986.

DECLARATION

I undertake:-

  1. To arrange for any defects to be rectified promptly so that the vehicle is maintained in a fit and serviceable condition.
  2. To make sure that the vehicle is at all times insured against liabilities to third parties for the purposes for which I wish to use the vehicle under the permit (Check Insurance document).
  3. To ensure that all persons driving the minibus will be over 21 years of age and will be experienced drivers holding a full driving licence and have D1 on their License. For those that don’t have D1 on their licence they may still drive as long as they have had their License for at least 2 years and drive on behalf of a voluntary organisation on a voluntary basis.
  4. To require the prompt reporting by drivers of any defects which come to their notice.
  5. To arrange for preventative maintenance and inspection for defects to be carried out at regular intervals of time and mileage.
  6. To ensure that there is a current M.O.T. Certificate for the minibus if the age of the vehicle requires it.
