Elderly/Disabled Housing Services Coordinator

Reports to:Support Services Administrator.

General Description:

The Elderly/Disabled Housing Services Coordinator is a social service staff person responsible for the coordination of supportive services to elderly residents, especially those who are frail or at risk, and those non-elderly residents with disabilities, which services are designed to assist the resident with maintaining compliance with leasehold obligations and to enhance daily living. S/he is responsible for the coordination of services and for identifying and accessing resources beneficial to these residents.

Principal Responsibilities:

  1. Works with relevant Authority staff and community service providers to coordinate the provision of services and to tailor the services to the needs and characteristics of residents.
  2. Takes referrals from QHA staff and meets with and identifies residents in need of essential services to assist them in maintaining their obligations under their lease and assists with tenants accessing such services.
  3. Establishes a system to monitor and evaluate the delivery, impact, effectiveness, and outcome of support services being provided residents.
  4. Coordinates services with other Authority or support services programs.
  5. Meets with QHA staff and other applicable persons and agencies to ascertain information essential to assisting resident in resolving housing issues and obtaining the services necessary to maintain compliance with leasehold obligations.
  6. Works with QHA staff and other applicable persons, agencies and residents to obtain services necessary to assist with compliance with leasehold obligation or in locating, recommending and placing residents in more appropriate housing alternatives.
  7. Maintains written reports in case files and provides both written and oral reports detailing the activities of the position.
  8. Participates in the writing of grants designed to address identified needs.
  9. Acts as a liaison with the South Shore Elderly Service Supportive Housing Program and other programs, as needed.
  10. Participates in and facilitates meetings with relevant Authority personnel, residents, family members, service providers and other participants to discuss resident/service needs and to resolve conflicts.
  11. When a resident fails to maintaintheir leasehold obligations, to assist with the eviction of such resident, if deemed necessary by the Authority, including participating in the Authority’s grievance process.

12. Performs other related tasks as assigned by the Supportive Services Administrator.

Knowledge, Skills and Abilities:

  1. Working knowledge of the local elderly services and disabilities support systems and the programs and resources they provide in the community.
  2. Ability to work with governmental entities in the City and state.
  3. Ability to maintain effective working relationships with residents, their families, QHA staff, and service provider agencies and their staff.
  4. Understanding of regulations and statutes related to state and federal programs.
  5. Ability to deal with people in a sensitive and effective manner.
  6. Ability to maintain confidentiality of private information.
  7. Ability to work with low income people and people of different cultures.
  8. Ability to communicate effectively both orally and in writing.
  9. Ability to analyze data and develop effective strategies to address needs.
  10. Computer literacy and ability to work with word processing, spreadsheet, and data base programs.

Training and Experience:

A four-year college degree in social work or related field preferred or two-years of college education; minimum of two years experience working with the elderly and/or disabled in a direct supportive service capacity. Requires a valid Massachusetts drivers license and regular use of vehicle.
