VIDEO AGREEMENT
AGREMENT made this ______day of ______, 200____, between "JAY JAYS VIDEO & AUDIO
RECORDING SERVICE" herein called "Video Tech"______(herein after called
"Purchase").
IT IS MUTTALLY AGREED THE PARTIES THAT:
1. Jay Jays will furnish and Purchase will accept, for the Video service hereinafter described, the service comprising of Video or Audio Recording Service. The service will be provided on a continuous basis during the stated hour.
2. Names, address, and telephone # of engagement where service will provided:

6. In the event Purchaser desires that Jay Jays Video & Audio Recording Service provide extended services, said Video Tech. shall be provided a rate of $ ______per hour, additional to the total price above.
7. A $ ______non-refundable Reservation Fee is required to secure Jay Jays Video & Audio Recording Service for the service date(s). This amount shall be subtracted from the total price of the agreement.

8. Purchaser shall pay the balance of $ ______in full plus any additional extended service costs in line 6, when Video is deliver to the purchaser for service by cash, check or money order made out to John E Jones, Jr. or Linda M Jones of Jay Jays Video & Audio Recording Service.
9. Event cancelled will subject to the non-refundable Reservation Fee of $ ______. Unless date is rescheduled within one year of original date (subject to availability).
10 Set-up for Video Tech Please furnishes for Cameraman adequate space for set-up and filming service with power source. If function is outdoors please make sure that Cameraman had adequate space for set-up and filming. Please make sure Cameraman is able to get good view of all service.
11. This contract constitutes the entire agreement between______and Jay Jays Video & Audio Recording Service. This contract may not be modified unless amended in writing and signed by the parties hereto.
