Under the freedom of information act I would be grateful if you could provide me with a copy of the following:

Clerking proforma(s) currently used in your trust

Discharge checklist if used in your trust

Discharge policy or procedure that is followed in your trust

Procedure followed for the discharge of anticoagulation patients on warfarin

Measures taken by your trust in order to implement a productive ward including ward round (and associated documentation used to the effect)

You have provided the following clarification for the above request;

Clerking proforma (s) currently used in your trust – proformas for what activity/department?

Please provide a copy of patient clerking proformas used with A+E, Medicine, Ambulatory Care and Surgery. By clerking proforma I am referring to the document completed when a patient is referred to a designated specialty during their inhospital stay.

Please see the attached documents.

The Trusts discharge policies are currently under review so have not been attached.

Measures taken by your trust in order to implement a productive ward including ward round (and associated documentation used to the effect) – please advise in what context you mean ‘productive’

By productive I mean focuses on improving ward processes and environments to help clinicians spend more time on patient care and at the same time improve levels of safety and efficiency.

‘Well organised ward elements’ of the Productive Ward Series have been implemented across the trust.

If you are unhappy with this decision then please do not hesitate to contact me. Your concerns or complaint will be fully investigated and a response sent to you within 20 working days, in exceptional circumstances this may take longer.
