Michigan College Scholarship -- 2016 Application

$1,000 (each) to Male and Female Lacrosse Scholar-Athletes, graduating high school in 2016. The number of scholarships will be determined by the committee based on number and quality of applications and availability of funds.*

Payment will be made to the student upon proof of college enrollment.

Application must be postmarked by May 13, 2016

Please supply two references, one coming from your 2016high school lacrossecoach or coach of a club you’ve played on, plus one other coming from a person who knows you personally (but not one of your 2016 lacrosse coaches and not a family member). Have your references send their letters to the address at the bottom of this application. References must be postmarked no later than 5.13.16. It is your responsibility to meet this deadline.
