Healthy Living Hero Award Nomination Guidelines

The La Crosse Healthy Living Collaboration (HLC), formed in 2009, consists of members of the La Crosse County community, area schools, public health, private businesses, health care and family serving agencies working to make La Crosse County and its residents the healthiest county in WI. The mission of the HLC is to make living tobacco free, eating healthy and being physically activity the norm among La Crosse County residents. Healthy Living Hero awards are a way for the HLC to recognize extraordinary efforts in the community to promote living healthy. Healthy Living Hero awards will be selected on an annual basis beginning in 2014.

Type of Nomination: Healthy Living Hero Awards are available in three categories; youth (age 18 and under), individual and organization. Awards will be distributed depending on the applications received.

Award Criteria: Healthy Living Hero Awards should showcase exceptional effort of the youth, individual or organization to meet or exceed efforts promoting the Healthy Living Collaboration’s goal areas to make living tobacco free, eating healthy and being physically activity the norm among La Crosse County residents

Application Rules:Nominations for the Healthy Living Hero Awards can be made by the individual seeking the award or by another person or group seeking to highlight the exceptional effort made by the youth, individual or organization.

Award Process: Complete the attached application with supporting materials as indicated with a due date of November 30, 2015. The HLC will form a review committee, comprised of current members. The committee will review application materials submitted and select Healthy Living Hero Award winners. One winner will be selected per designated category, depending on the awards nominations received. Healthy Living Heroes will be invited to the first La Crosse Healthy Living Collaboration meeting January 8, 2016 to receive their award. The media will be invited to the meeting to announce the award winners. The process will be completed on an annual basis.
