Parental and Participant Consent Form for Youth Ministry Events
The following is a list of rules and guidelines that are expected to be adhered to during the Event. We would like all participants to remember that at all times they are being representatives not only of themselves, their parents and their schools, but also they represent Grace United Methodist Church—and above all God Almighty.
1. The leaders of the Event will know where the participant is at all times during the trip.
2. All participants must remain in the Event area at all times.
3. When a participant requests to be involved in activity away from the main group that participant must have at least one "buddy". The participant will remain with their "buddy" at all times except for showers and bathroom trips until they return to the main group.
4. No members of the opposite sex are allowed in the participant’s sleeping area.
5. All participants are required to have a good time.
I, the undersigned, agree to the rules and guidelines of the Event. During the Event, I will behave in such a manner as to bring honor to my parents and my peers. I will make this Event the best it can be for all other participants. I understand that if I break this covenant I will suffer consequences that may include being removed from the event.

We (I), the parent(s) or legal guardian of the above-mentioned participant do agree with the rules and guidelines of the Event. If the participant named above, or his or her guest, breaks the covenant, we (I) agree that the Grace United Methodist Church Youth Leaders may determine the appropriate consequences for breaking the covenant. Further, We (I) agree to pick him/her/them up as soon as possible should his/her/their participation in the Event be suspended as consequence for poor behavioral choices.
