Mosaic Community Care Inc. (Mosaic) provides supports which are tailored to suit each person’s individual needs and aspirations at various stages of their life. Our services include:
· Supported Accommodation: Shared living in purpose-built accommodation (or independent living units) with a dedicated, experienced team providing 24/7 support.
· Individualised Services: Flexible and person-centred supports within the individual’s home or within the community.
· Positive Behaviour Support: Fully qualified and dedicated specialists available on hand to gain a better understanding of the individual’s behavior and to develop effective strategies in order to reduce and eliminate challenging behaviours and restrictive practices over time.
· Justice Program: A diverse range of community-based supports and interventions for a person with a recognised disability who may be interfacing with the police, courts and justice system.
· Live-In Care / Co-Resident: 24/7 individualised and person-centred supports provided by an experienced support worker to assist a person with a disability to live in their own home and build and maintain connections within their community of choice.
· Shared Management: Opportunities available upon request.

Individuals and their family or other stakeholders make independent decisions about which supports or services they wish to purchase and this guide is provided to support that process.
