Guide to Completion for the Cultural Resources Site Report, FL-CPA-46
The Cultural Resources Site Report, FL-CPA-46 may be printed out or completed in its electronic form and forwarded to the NRCS SO-ECS CRC andCRS. Both atopographic and aerial map of the APE, must accompany the FL-CPA-46.
1.Name of Planner – Name of planner that has obtained certificate of completion for the Cultural Resources Training Modules (1-8).
2. Date – Date of form completion, (automatically filled with Excel function).
3.Tract# – For filing and reference purposes.
4.Field# –Field number(s) corresponding to APE(s).
5.Anticipated Start Date – Indicate month and year of when installation is anticipated.
6.Site(s)#–Reference number of cultural resource site as indicated on GIS cultural resource layers, if applicable. Site # is not applicable, if site is unknown and only observed during field inspection.
7.Program Activity – Indicate what program is funding the undertaking.

8.USGS Quad Name– Name of quadrangle where site is located.
9.County –Name of county where site is located.
10.Township, Range, Section– Name of T,R,S where site is located.
11.UTM Coordinates – Easting, Northing of location of site’s approximate center.
12.Site Dimensions – Greatest width and length (in feet) of site.
Planned Conservation Practices
13.Potential to Affect (P) or Low Potential to Affect (L)– Choose those practices from drop down list that are being planned. If practices are not listed in drop down list, use two bottom lines to describe or name measures that are proposed to be installed.
Area of Potential Effect (APE) Description
14.Ground Cover – Approximate percentage of ground cover; if more than one ground cover type is present, all percentages should equal 100%.
Example:Forest 45%; Range/Pasture 20%; Crop; 35%
15.APE Surface Visibility % – This is the approximate percentage of soil or exposed ground visibility.
16.APE (Acres) – Approximate acreage of specific practice area (indicate feet, if linear). The APE does not constituent the entire farm or tract acreage unless the practice comprises the entire area.
17.% of APE Inspected – Percentage of APE surface inspected.
18.Soil Type–Need soil type and association.
How Did You Discover the Cultural Resource(s)?
19.Check any and all options that apply.
Describe Cultural Resource(s) found, if any, during Field Inspection.
20.Provide description of material and item itself such as ceramic pottery, metal nail, elevated ground, depressional area, etc. Include any photographs of the cultural resource(s). Include UTM coordinates in upper portion of this report of any cultural resource(s) observed.
The remainder of the report is the NRCS SO-ECS CRC/CRS Review section which can only be completed by the NRCS SO-ECS CRC/CRS.
