Overview

Beginning in April 2018, Medicare will begin paying for certain diabetes prevention lifestyle change programs that have been recognized by the Centers for Medicare and Medicaid Services (CMS) and the Centers for Disease Control and Prevention (CDC). The purpose of this FHI 360 project is to obtain feedback about CDC messages and information about this program, and whether these messages and information may encourage Medicare Part B beneficiaries to talk to a health care professional or contact a lifestyle change program to possibly join.

FHI 360 is requesting assistance with 12 focus groups, four in each of three locations in the U.S.: Memphis, Tennessee; Philadelphia, Pennsylvania; and Riverside, California. The applicant organization will recruit participants; collect (focus group facilitation) and analyze data; transcribe discussions; participate in discussions about findings; and handle all other logistical aspects of implementing the stated activities (e.g., facility rental, incentive payments, reporting) at the three locations, according to the parameters outlined below.

Description

The purpose of conducting focus groups with Medicare Part B beneficiaries is to explore participants’ thoughts on various messages and materials by asking participants to review the messages and materials and share their opinions. The focus groups discussions will help CDC to further develop messages to be used in print and online materials to reach Medicare beneficiaries with information about the lifestyle change programs and encourage participation.

Scope of Work

·  Recruit and provide FHI 360 with a list of recruited participants who represent a mix of:

o  Age (65-69 years and 70-75 years)

o  Race/ethnicity

o  Level of education

o  Working and retired

o  Prediabetes (diagnosed) and being at increased risk for type 2 diabetes without diagnosed prediabetes

·  Conduct 12, 90-minute focus groups (eight respondents per group). Groups will be segmented by gender at each location:

·  Provide and collect consent forms from each participant.

·  Provide professional moderation for each focus group.

·  Provide and distribute incentives of $75 per focus group participant.

·  Provide audio recording and transcription of each focus group.

·  Provide remote viewing.

·  Participate in planning discussions, post-group discussions and a discussion to talk about findings after the conduct of all the focus groups

Tasks/ Deliverables

1.  Participation on calls with FHI 360 and/or the CDC

2.  Draft activity implementation plan

3.  Recruit focus group participants for each site

4.  Facilitate focus group for each site

5.  Record and provide video streaming of focus groups (real time)

Proposal Submission Requirements

All responses to this RFP must be submitted electronically and received no later than 05:00 PM EST Monday February 19, 2018 to Kaly Moot, in either Word or PDF format.

Offerors should read the following proposal instructions carefully. All interested offerors must provide the following:

A.  A brief description of the organization, its capabilities, and its ability to implement focus groups.

B.  Resume/CV of proposed focus group facilitator (single person)

C.  Overview of past experiences recruiting and/or working with target population of 65+ year old individuals.

D.  If available, overview of past experiences working in specified markets of Memphis, Tennessee; Philadelphia, Pennsylvania; and/or Riverside, California.

E.  Budgets must be sufficiently detailed to demonstrate reasonableness and completeness and include a breakdown by scenario, type of cost, and unit value and quantity. Budgets should also include detailed line items. Offerors including budget information determined to be unreasonable, incomplete, unnecessary for the completion of the proposed project, or based on a methodology that is not adequately supported may be deemed unacceptable.

F.  In the budget, the offeror must clearly show the cost to FHI 360 for services provided by the firm. Offeror should clearly show if those costs are calculated as a percentage of the value of the media buy or as a flat fee. If the fee is calculated as a percentage, offeror should indicate if the percentage charge is calculated on the gross or net value for the media time purchased. Please also explain how the fee will be calculated if the total value of the media buy changes from the specifications in this RFP.

G.  Three references for a similar scope of work.

H.  FHI 360 may contact bidders and/or their client references by phone or email for clarification of past media buys or other aspects of their work prior to the award. Please provide contact information with your submission.

I.  Any additional considerations that you believe would provide added value to this work.

J.  Your proposal should not exceed 5 pages in length. Additional pages to the proposal will not be reviewed. A cover page for the proposal and detailed budget will not be counted in the 5-page limit.

Criteria for Evaluation

Proposals will be evaluated and ranked by FHI 360 committee according to the conditions described in the evaluation criteria below, with a total overall maximum point value of 100:

Selection will be based on “Best Value.” The subcontract shall be issued to the offeror with the highest combined technical and cost score.

FHI 360 will not compensate offerors for its preparation of its response to this RFP.
