TITLE V INFORMATION Instructions for Form E1
Form E1 contains general information on the Title V facility being permitted. One Form E1 and all subsequent “E” forms (E2 through E4) must be completed for each Title V permit application.
MAJOR FOR TITLE V BY CATEGORY OR EMISSION LEVEL - Indicate if your facility is major for Title V by category or emission level. 40 CFR 70.2 and MCAPCO 1.5502 define a major facility by either category (type of facility) or emission level. Indicate whether your facility is major by category or emission level as defined in MCAPCO 1.5502.
EMISSION LEVEL - If the facility is major by emission level, list the pollutants for which the facility exceeds the emission rate established for a major facility as defined in 40 CFR 70.2 or as designated by the EPA. Also, indicate the rate (including units) at which the pollutant(s) is/are being emitted by your facility.
CATEGORY - If the facility is major for a category, indicate the category(ies) which apply(ies) to your facility as designated by the EPA.
MAXIMUM ACHIEVABLE CONTROL TECHNOLOGIES (MACTs) - Indicate all promulgated MACTs or pending MACTs as listed in 40 CFR Part 63, for which your facility is subject or will be subject.
PERMIT SHIELD - All regulations in the permit application will be subject to a permit shield. List here any additional regulations for which you are requesting a shield and explain. A permit shield is clearly defined in MCAPCO 1.5512.
