Diabetes Nurse Educator

$60,790 - $71,518($33.40 – $39.30 per hour) + Health and RRSP Benefits
The Vaughan Community Health Centre is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the City of Vaughan. In particular, we serve those people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. As part of the health promotion framework of our services, we work in partnership with other community-based health and social service organizations to address individual, family, and community needs.
POSITION SUMMARY:
As a member of the interdisciplinary team, the Diabetes Nurse Educator provides diabetes education to CHC clients and community members through individual and group counseling. The Diabetes Nurse Educator works within a collaborative and inter-disciplinary model of care to serve the adult type 2 diabetes population and provides self-management education and support for the development and implementation of accessible and culturally relevant programs that address barriers to self-management education and practices.
RESPONSIBILITIES:
- Provide assessment of clients who are at risk and/or have pre-diabetes and diabeteswithin the nursing scope of practice taking into account patient’s level of knowledge, readiness to learn, socio-economic and cultural background
- Develops an individual health maintenance plan in collaboration with client and other healthcare providers
- Works with the Diabetes Team to plan, implement, facilitate, and evaluate innovative diabetes group education sessions in the community
- Recommends appropriate referrals to primary providers and allied health professionals within the community and provides follow-up with individuals as necessary
- Ensures continuity of care by maintaining accurate, and concise medical records, monitoring clients on an as-needed basis, clinical case management meetings, and communicating with the client’s primary care provider and other service providers as required
- Enters client data on the Electronic Medical Record system. Participates in chart reviews
- Develops, obtains and evaluates diabetes education resources and counseling materials
- Outreach in collaboration with other VCHC staff to identify target populations and to locate and book community locations as host sites for group workshops
- Establishes and maintains relationships with diagnostic company representatives with regards to current diabetes supplies and teaching materials
- Demonstrate the capacity to advocate and support culturally diverse communities and identify current gaps of service
- Supervise and mentor students/interns.
- Maintain confidentiality and professional-ethical standards of conduct at all times
- Provide timely information and reports, as required by VCHC and/or program coordinator
- In collaboration with the diabetes team, perform needs assessment and plan, develop, implement, promote and evaluate the community-based diabetes education program for York region's diverse high-risk population
- Plan, organize, coordinate and engage actively in health promotion activities towards diabetes management, by participating in outreach activities and presentations in targeted population areas on and off sites in conjunction with other service providers
SKILLS AND QUALIFICATIONS:
- Bachelor of Science in Nursing Degree
- Registered Nurse in good standing with the College of Nurses of Ontario
- Maintaining or working towards Certified Diabetes Educator (CDE) status through the Canadian Diabetes Educator Certification Board (CDECB) ( within 2 years)
- Knowledge and experience in insulin therapy is an asset
- 2-5 years of experience in clinical counseling, community based diabetes education or equivalent
- Demonstrated current knowledge of evidence-based best practice and diabetes clinical practice guidelines
- Demonstrated skills in adult learning and group facilitation.
- Demonstrated ability to work effectively in a multi-disciplinary team
- Familiar and comfortable with co-counseling skills
- Experience with conflict resolution techniques
- Excellent communication skills and adaptable to change
- Commitment to health promotion strategies and philosophy
- Sensitivity and awareness of multi-needs of marginalized populations, including seniors, newcomers, people with mental health issues, persons with disabilities
- Cross-cultural experience and a second language reflective of the community groups represented in Vaughan i. e. Urdu, Italian, Russian, Spanish, Hindi, Punjabi an asset.
- Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues in diabetes self-management
- Experience with outreach and community-based health care
- Current CPR certification ( Within 2 years)
- Valid Driver’s License to drive in Ontario and access to a vehicle
The position will involve working regular office hours, evening and weekends. Successful candidates must undergo a Vulnerable Sector Screening reference check to the satisfaction of the Vaughan Community Health Centre.
