Stated below is Blue Knobs policy on missed skiing days: "There are no refunds, with or without doctor excuses. If a student is ill and can not attend, it is up to that student to find a replacement and collect the money from his replacement. The Replacement must have a signed permission slip from their parents/guardian by the Friday before the date scheduled.

Make sure both permission forms (CENTRAL PARENTAL PERMISSION FORM AND BLUE KNOB RENTAL/RELEASE FORM) are signed and circle the ski package you desire. Anyone renting a snowboard must also sign the snowboard release form.

Please place payment and signed form in an envelope to avoid any mix up.

Students must ride the bus to and from Blue Knob unless a parent or guardian picks up the student personally and notifies the Ski Club advisors.

Central High School Journey Parental Permission Form

My child, ______, has my permission to go on a school journey to Blue Knob on Jan. 5, 12,19, and 26 supervised by faculty members. While on this journey, I absolve, release, and exonerate the Spring Cove School District and all its employees from all claims, financial or otherwise, due to any injury or damages incurred by the above - named child while on the trip designated. In the event of inclement weather, the school authorities reserve the right to postpone this trip to another date without in anyway invalidating any of the terms agreed to on this form by the parents or guardians of the student named above.

BLUE KNOB RENTAL AND RELEASE/LIABILITY FORM

PLEASE READ CAREFULLY BEFORE SIGNING

I accept for use as is, the equipment listed on this form and accept full responsibility for the care of the equipment while it is in my possession.

I will be responsible for the replacement, at full retail value, of any equipment rented under this form which is not returned to the shop and I acknowledge my obligation to return this equipment by the agreed date in clean condition, to avoid any additional charges.

I agree to hold harmless and indemnify the Blue Knob Rental Shop, as well as its owners, agents, employees, for any loss or damage, including any that results from claims for personal injury or property damage related in any way to the use of this equipment – except reasonable wear and tear of the equipment itself.

I understand there are inherent and other risks involved in the sport of skiing, for which this equipment is to be used, and that injuries are a common and ordinary occurrence of the sort and I freely accept these risks.

I understand that the board/boot/binding system will not ordinarily release during use, nor is it specifically designed to release as the result of forces induced during ordinary operation, and is therefore absolutely no guarantee of my safety.

In consideration of all the above and also in consideration of the equipment being rented to me or at my request, I hereby release Blue Knob Resort and its owners, agents, and employees, from any and all liability of damage and adjustment, and use of this equipment, accepting myself full and complete responsibility for any and all such damage or injury or death which may result.

I have made no misrepresentations to Blue Knob Resort with regard to any information which they have requested or related to the rental and/or use of the equipment.

Full instructions on the use of this equipment have been made clear to me, and I understand the use and function of my equipment.
