Substitute Teacher Request for Consideration Form
Dear Applicant:
Thank you for your interest in becoming a substitute teacher for SOS Personnel. The requirements for becoming a substitute teacher are listed below.
- A teaching certificate (if you have one)
- Proof of a Degree
- FBI Clearance
- Child Abuse Clearance
- State Criminal Record Check
- Recent TB Test or Chest X-Ray that is less that 2 years old
- A Resume
- An SOS application form (can be found on the SOS website)
- 2 Professional letters of reference
- A valid form of ID
- A completed W-9 form
Your request will not be considered complete until all of the above requirements have been met. Please allow 2 weeks for it to be processed. If your request is approved, you willbe contacted by email or phone.
