Substitute Teacher Request for Consideration Form

Dear Applicant:

Thank you for your interest in becoming a substitute teacher for SOS Personnel. The requirements for becoming a substitute teacher are listed below.

  • A teaching certificate (if you have one)
  • Proof of a Degree
  • FBI Clearance
  • Child Abuse Clearance
  • State Criminal Record Check
  • Recent TB Test or Chest X-Ray that is less that 2 years old
  • A Resume
  • An SOS application form (can be found on the SOS website)
  • 2 Professional letters of reference
  • A valid form of ID
  • A completed W-9 form

Your request will not be considered complete until all of the above requirements have been met. Please allow 2 weeks for it to be processed. If your request is approved, you willbe contacted by email or phone.
