NB: If you are leaving during the school day please report to the school office to meet your parents and sign out.

This form must be completed for students seeking Permission for Absence from school for the following if they are known in advance:

·  Doctor’s appointment

·  Dental Appointment

·  Hospital appointment

(Please provide evidence for the above appointments.)

·  External Exams taken away from school (Music, Dance etc.)

·  University Open Day visits ( only 3 permitted during term time)

·  Holidays/ Occasional days leave ( See Notes below)

As much advanced notification of any proposed absence is extremely desirable. This is not only a matter of politeness, but will ensure that your child’s work at school suffers minimum disruption.

This form must be completed by a parent/guardian of the student concerned and returned to the Headmaster’s Secretary (in Room O11) as soon as possible. The reply slip will be returned to you with a decision.

Notes

1. The law states that “Headteachers may not grant any leave of absence during term time unless there are exceptional circumstances.”

2. Any absence affects the normal progress of learning. Students are only in School for 190 days a year.

3. The School wishes very much to discourage parents from removing their children from school for occasional days. Such absence may well be regarded as unauthorised by the Headmaster.
