Dear Parents:
A Harmon Brothers charter bus has been rented to transport Chiefs fans to Columbus, GA on Friday, February 26th for the Elite Eight boys basketball game. Reservations for the charter bus travel will be accepted Tuesday, February 23rd through Thursday, February 25th during lunch; the school parent permission to travel on the charter bus and a $23 transportation fee are required at the time of reservation. Please note, school rules apply to students traveling on the charter bus. Refunds will not be available unless the charter bus is canceled due to lack of interest, inclement weather, or state/national emergencies. If you have any questions concerning this field trip, please contact Ms. Stacey Smith at McIntosh High School.

CHARTER BUS PERMISSION FORM: I give permission for my child to travel on the school-secured charter bus. I understand that the scheduled charter bus could be canceled and the $23 fee will be refunded, if student interest does not support the need for a bus. Additional reasons for cancellation of the charter bus could include but are not limited to: inclement weather, state or national emergencies, or exorbitant fuel costs. I agree not to hold the Fayette County Board of Education responsible should the field trip have to be canceled and funds not refunded.

CONSENT AND RELEASE:I hereby consent on behalf of the student named above to participate in the school-secured charter bus transportation to Columbus State University. If any emergency medical procedure or treatments are required by the student during the trip, I consent to the trip’s supervisor taking, arranging for, or consenting to the procedures or treatment in his or her discretion. I further release and waive and further agree to indemnify and hold harmless and reimburse the Fayette County School System, the Board of Education, its successors and assigns, its members, agents, employees, and representatives thereof, as well as the trip supervisor from and against any claim which I, any other person, firm, corporation, or entity may have or claim to have, known or unknown, directly or indirectly, from any losses, damages or injuries arising out of, during, or in connection with the student’s participation in the activity, any trip associated with the activity, or the rendering or emergency medical procedures or treatment, if any.
