Award applicant must:

A. Be a current member of New York State AHPERD.

B. At the time of application, be an undergraduate junior or senior with an overall GPA (3.0 or above) at the New York State college/university that he/she attends.

C. At the time of the award presentation at the Annual Conference (November 20–23, 2013), be an undergraduate senior within two (2) semesters of graduation, but not post graduation.

D. Be majoring in the field of Health, Physical Education, Recreation, or Dance.

E. Fully complete and sign the application and arrange for 3 letters of recommendation.

F. Verify participation in professionally related activities (See Rubric).

G. Demonstrate, via the application process, an exceptional interest in, involvement with, and commitment to his/her chosen field. (See Rubric).

H. Be selected as his/her college/university Departmental Major of the Year.

CANDIDATE’S STATEMENT: Please tell us about yourself: your character, your accomplishments, your talents and why you feel you deserve this award. Explain and further clarify your involvement in the professional activities noted in your application. Describe your professional goals after graduation, both immediate and future. Type your statement on a separate paper. Use double spacing.

REFERENCE LETTERS: Three (3) reference letters must be included with the application. At least two (2) must be from a teaching faculty at your current college/university. One (1) may be from another source related to your future profession. Sources must state in the letter how they know you. Please use the attached form for the recommendations. Make copies of the form as needed.

NOTE: All reference letters should be on official letterhead and in sealed envelopes with signatures across the seal or emailed to the Department Chair or designee.

STATEMENT OF ACKNOWLEDGEMENT

I UNDERSTAND THAT THE FOLLOWING CONDITIONS OF ACCEPTANCE MUST BE MET:

If selected as Departmental Major of the Year, I will:

1. Attend the NYS AHPERD Conference in November of my senior year.

Be recognized as my school’s Departmental Major of the Year at the Amazing Persons Awards Ceremony on Thursday evening of the conference. Lodging for two days will be paid for by the Future Professionals Section.
Attend the Jay B. Nash Awards Ceremony on Friday evening of the conference.
Serve as Presider at one session sponsored by the Future Professionals Section
Participate in the “Following the Leader Program” sponsored by the Future Professionals Section at the conference.

In addition to the above, if selected as the Jay B. Nash Outstanding Major of the Year, I will:

1. Be recognized as the Jay B. Nash Outstanding Major of the Year at the Jay B. Nash Awards Ceremony. Lodging and a meal stipend for two days at the association per diem rates will be paid for by the Association. Registration fee is waived.

2. Join the American Alliance for Health, Physical Education, Recreation and Dance (AAHPERD) if not already a member.

3. Attend the Eastern District Conference in the spring of my senior year as the Outstanding Major from New York State. EDA will provide three nights of lodging.

4. Maintain membership in NYS AHPERD in order to be eligible to receive the monetary award.

5. Verify NYS AHPERD membership, payment and completion (official transcript) for course work to receive remuneration.

If I fail to meet the conditions of acceptance or do not matriculate during the award year, all award benefits will be forfeited.
