Applications are invited for a Specialty Grade Doctor in Paediatric Haematology and Oncology. This whole time post is based at the new Royal Hospital for Children (RHC) on the Queen Elizabeth University Hospital (QEUH) site , where it is co-located with a new adult hospital and a re-developed maternity hospital. This is a replacement post to cover maternity leave and is available from as soon as possible

Initially for a period of 11 months.

The main responsibilities of the post holder will be to undertake clinical duties relating to inpatients, outpatients and day care attendees along with a team of middle grade trainee doctors and FY2s. The successful applicant will take part in the haematology and oncology middle grade rota as a fixed commitment. The post-holder will be expected to participate in the undergraduate, postgraduate and nurse training programmes.

The appointee will contribute to the existing team of 4 Specialty Grade Doctors, an Associate Specialist and 8 Consultants within the department.

The successful candidate should have experience at middle grade level in medical paediatrics. Experience in haematology and oncology would be an advantage, but is not essential.

All applicants must possess Full GMC Registration with at least 4 Years full time Post Graduate Training, two of which must have been within a Specialty Training programme, or should be able to demonstrate and evidence equivalent experience and competencies.

Section 1 Royal Hospital for Children

Children’s Services across NHS Greater Glasgow & Clyde

The Royal Hospital for Children (RHC), Glasgow, is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow, but is also a tertiary referral centre for children from the West of Scotland and, in some subspecialties, from the whole of Scotland. It has 266 inpatient beds, 12 daycase beds, and handles approximately 90,000 out-patients, 15,000 in-patients, 7,300 daycases and 35,000 A&E attendances every year. The hospital provides care for children from newborns up to around 16 years of age.

All paediatric medical and surgical subspecialties are represented, including general medical paediatrics, cardiology, neonatology, neurology, neurosurgery, nephrology, respiratory, endocrinology, gastroenterology, immunology and infectious diseases, dermatology, haematology, oncology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, orthopaedics and general paediatric and neonatal surgery. The critical care service includes a high dependency unit, an intensive care unit and a combined neonatal medical and surgical intensive care unit, as well as a paediatric and neonatal retrieval service. The hospital provides a national ECMO service and is the centre for all paediatric cardiac surgery, bone marrow transplantation and renal transplantation in Scotland. A selection of child and adolescent psychiatry facilities are located within the campus.

In its role as a major academic institution, RHC is home to a number of University departments as well as internationally acclaimed research groups. The significant commitment to the teaching and training of new doctors, nurses, midwives and other health professionals ensures that highly trained NHS staff are ready to care for the children of tomorrow. It provides the major undergraduate paediatric teaching facility for the University of Glasgow.

The diagnostic imaging department located within RHC provides ultrasound, CT, MRI and isotopic studies on site. Diagnostic laboratory facilities on the site include haematology, blood bank, biochemistry, microbiology, virology, histopathology and genetics . Cytogenetics and diagnostic molecular haematology and oncology are located on this site.

Other Paediatric Services in NHS GG&C

There are currently three maternity hospitals in Glasgow/Paisley: Princess Royal Maternity Hospital (PRMH), Queen Elizabeth University Hospital (QEUH) and the Royal Alexandra Hospital (RAH); all have neonatal facilities.

There is one children’s ward at the RAH providing care locally or the children from Paisley.

There is an extensive range of specialist community based children’s services across NHS Greater Glasgow. Managed within community health and social care partnerships, these services are integrated with primary care and social care services.

Section 2 Paediatric Haematology and Oncology Services

The clinical Haematology / Oncology service is part of the Women and Children’s Directorate of NHS GG&C providing care for children with leukaemia, solid tumours, brain tumours, bleeding disorders, haemoglobinopathies and a range of benign haematological conditions from the West of Scotland. It houses the National Paediatric Haemopoietic Stem Cell Transplant (HSCT) Unit and is the Regional Paediatric Haemophilia Centre. The department is a Children Cancer and Leukaemia Group (CCLG) Centre and eligible patients are entered into appropriate NCRI protocols.

In-patient Facilities

There is a dedicated Haematology / Oncology Unit (Ward 2a) with 25 inpatient beds, all of which are individual cubicles with resident parent/carer facilities and 4 HEPA-filtered rooms for HSCT. There is a comprehensive Day-care facility (Ward 2b) with consulting rooms and 8 day-care beds/reclining chairs. There is parent accommodation in the nearby CLIC-Sargent supported Marion House and Ronald McDonald House. There is a small classroom with provision for both primary and secondary education and small play room supported by play therapists. Pharmacy support is provided by the dedicated oncology pharmacists based on and near the unit. The electronic chemotherapy prescribing system (Chemocare) is in use with ongoing development. The Unit is JACIE and HTA accredited. The unit is actively recruiting eligible patients for NCRI and commercial trials and is a recognised ITCC centre.

A 4 bed adolescent unit funded by the Teenager Cancer Trust with additional two beds for adolescents and generous social space are located within Ward 2a. .

Middle grade staff do not participate in the general medical paediatric on-call rota, but provide dedicated out-of-hours cover to the Haematology / Oncology Unit up to 21.30hrs in the evening including the weekends. Overnight cover is provided by the hospital at night team.

National Arrangements - Shared Care

Stem Cell Transplant is funded by the National Service Division (NSD). Paediatric Oncology and Malignant Haematology is now a national service supported by the Managed Service Network (MSN) for Children & Young People with Cancer. Scotland has three treatment centres and three shared care centres. Shared care arrangements currently exist between the RHC Glasgow and local District General hospitals in Dumfries and Inverness. Within this arrangement, in-patient based chemotherapy and intensive aspects of chemotherapy are delivered at RHC Glasgow with outpatient reviews and supportive care provided locally. Shared care clinics are held every 3 months for haematology and every 6 months for oncology in Inverness. Shared care units provide support, including agreed investigations (blood counts) and treatment (febrile neutropenia, blood product support) nearer to the child's home and have an important role in the delivery of palliative care.

There are designated Consultant Paediatricians in these centres and paediatric outreach nurse..

Existing shared care arrangements are under review.

National MDTs for leukaemia take place weekly and for solid tumour fortnightly. National palliative care MDT takes place every two weeks either by tele or videoconference. A national blood transfusion policy and a national guideline for febrile neutropenia are in place and there is a national educational programme for trainees which is conducted by V/C.

Section 3 Duties of the Post

The successful applicant will have responsibilities in all areas of clinical care, including ward, day-care and outpatients. The post holder, after appropriate training will also undertake responsibilities such as chemotherapy prescribing , performing lumbar punctures , administering intrathecal chemotherapy , performing bone marrow aspirations and trephine biopsies. He/she should have clinical paediatric experience. He or she is expected to work fairly independently but with supervision, support and advice available at all times from seniors. He/she will be responsible for the administrative duties associated with the care of patients and will participate in the on-call service.

Out of hours work will comprise an evening session 1 in 6 (17:00 – 21.30hrs) and one weekend in 6 (Friday 17:00 – 21.30hrs and Saturday/Sunday 09:00 – 21.30hrs). No overnight cover is expected either during the weekdays or weekends.

Research & Development

The successful candidate will be expected to maintain personal professional development appropriate to the post. There will be opportunities for individual or collaborative research and development within the service.

Teaching

The successful candidate will be expected to engage in undergraduate and postgraduate teaching, appropriate to experience.

Proposed Job Plan

The post holder will be one of 4 Specialty Doctors who will provide continuity of care across all aspects of the service.

The following work plan can be adjusted as needed, depending on personal and departmental needs.

The contract is for up to 10 Programmed Activities.

These 10 sessions would consist of:

Inpatient, outpatient, day-care work and procedures 7 sessions

8 week-ends per annum (1 in 6) 1.6 sessions

1 in 6 late shifts 0.4 session

Supporting professional activity eg. teaching, administration 1 session

and audit

The actual timing of these sessions is negotiable, but it should be reiterated that there must be considerable flexibility.

Facilities and Education

A number of educational hospital meetings occur regularly, including an afternoon dedicated to continuing education which is accredited for CPD purposes. There is a weekly departmental educational meeting which takes the form of case presentations/journal club.

General

All medical and dental staff employed by the Acute Services Division are expected to comply with all Hospital Health and Safety Policies applicable on the various hospital sites.

The successful candidate will be expected to organise his/her workload and operate an appointment system in a manner that is consistent with good practice.

The post-holder will be expected to undertake the administrative duties necessary to the tasks listed above, to the care of his/her patients.

The post-holder will be expected to establish and participate in audit projects involving the service delivery of the departments and be aware of clinical governance and clinical risk management.

The post-holder will be expected to contribute to the management of the department at a level commensurate with experience and to co-operate with operational policies of the department and directorate.

Managerial

The management responsibility of the post-holder will be to the Link Clinician for Haematology and Oncology Services, Women’s and Children’s Directorate (current post holder is Prof Brenda Gibson).

Administrative Support

Requirements:

The applicant must have full registration with the General Medical Council, a licence to practise and produce evidence of this prior to commencing the role.

This is a non-training grade.

The applicant should have completed at least four year’s full-time postgraduate training (or it’s equivalent gained on a part-time or flexible training basis) at least two of which will be in paediatrics or paediatric haematology /oncology. Additionally, the applicant should have sufficient experience to exercise an intermediate/middle grade level of clinical responsibility delegated by the Consultant.
