Your shifts will be variable and will include a range of Local Treatment Centres, Triage and Visiting shifts /TITLE OR POSITION
NORMAL PLACE OF ATTENDANCE
All settings linked with the Urgent Care Service and including patients own place of residence(Part of course/secondment may take place elsewhere with discussion and agreement with your GP Trainer and the Director of Urgent Care and Clincial Services.

SICKNESS/ABSENCE
If you are sick or absent for any reason, you (or someone on your behalf if you are unable to do it yourself) will notify your ClinicalSupervisor/Trainer and UCS Administrator, as soon as possible on your first day of absence.PERSONAL HEALTH
As a public health organisation, the Trust takes its responsibilities for health promotion very seriously. In consequence, it has adopted policies related to personal health such as no smoking and alcohol on Trust premises and immunisation, copies of which can be obtained from the Ambulance Trust’s Human Resources department.LOSS OF PROPERTY
The Trust cannot accept responsibility for articles of personal property, lost, stolen or damaged, including motor vehicles on its premises.RESEARCH GOVERNANCE
South Western Ambulance Service NHS Trust manages all research in accordance with the requirements of the Research Governance Framework . As a contractee of the Trust you must comply with all reporting requirements, systems and duties of action put in place by the Trust to deliver research governance. /PERFORMANCE
The Trust will have absolute discretion to terminate your placement/secondment etc at any time for misconduct or any other substantial reason including in the interest of patient/client care. This would occur in consultation with the GPR, GP Trainer, the School of General Practice and the Director of Urgent Care and Clinical Services. The GPRs would be eligible to appeal through the Trusts Human Resources department.CONFIDENTIALITY
The Trust requires that you maintain patient and individual confidentiality at all times. Failure to do so will result in immediate termination of this placement/secondment etc.
ACCIDENTS IN THE WORKPLACE
If you are involved in an accident on Trust premises, you must inform the person to whom you are accountable immediately.
HEALTH AND SAFETY AT WORK ACT 1974
The Trust is aware of its obligations to safeguard the health and safety of its employees, visitors and patients under the provisions of the Health and Safety at Work Act (1974). Your attention is drawn to the written Health and Safety Policy, which is available from the manager or supervisorDECLARATION OF POLICE INVESTIGATION, ARREST OR CONVICTION
If during the course of your placement/secondment you are arrested, charged or convicted of any offence you must notify the Directorof Urgent Care and Clinical Services as soon as reasonably possible. Such an occurrence will not automatically disqualify you from the placement but the Trust reserves the right to terminate the placement should we feel it appropriate following discussion with your GP Trainer and the Director of Urgent Care and Clinical Services.
COMPLAINTS
If you have a complaint arising from your placement/secondment with the Trust you should take this up with the Director of Urgent Care and Clinical Services. Should you remain dissatisfied please see the complaints procedure a copy of which is held at the UCS Call Centre. /
REGISTRATION
It is a requirement that doctors, qualified nurses, midwives and staff in the professions supplementary to medicine, will produce evidence of professional registration prior to commencement of this placement.OTHER TERMS
Save in so far as expressly set out in the foregoing clauses your terms and conditions shall be determined by the Trust as appropriate.The South Western Ambulance Service NHS Trust reserves the right to vary these terms and conditions with full consultation and approval with your GP Trainer and the Directorof Urgent Care and Clinical Services.
