1. District Student Contract/Agreement: Read but do NOT print this! Then sign part 2 of your info form.
2. Broadcaster’s Code of Conduct (Agreement): Do NOT print this! Read & then sign part 3 of info form.

___ Hand in the printed INFO FORM (complete and signed). (But also bring a digital copy on your USB drive)

___ High Road Music(You must have the music stored on something portable; being able to
access it online does NOT count for this requirement. Recommended: 10 or more songs on
a CD and 10 or more songs on a USB drive and 10 or more songs on your phone/MP3 player.
_____4. THERE IS A BAN ON USING PHONES AND PERSONALELECTRONICS IN OUR
CLASSROOM AND STUDIOS UNLESS GRANTED PERMISSION BY THE TEACHER FOR A SPECIFIC PURPOSE ON A
SPECIFIC DAY. (In sight will also be considered in use.) PHOTOS, FILMING and / or
RECORDING ANYTHINGwithout teacher permission IS PROHIBITED. TAKING, SHARING
(in any way) and/ or SENDING ANYTHING FROM OUR CLASS without teacher permissions IS
PROHBITED. NO online posting, texting, e-mailing, tweeting, etc. of anything or anyone from our class without teacher
permission, even if your actions occur out of class and even if it is your own work. The same policies apply to
classroom technology, as well. Use classroom technology ONLY with teacher permission and ONLY for specific, pre-
approved class business. When given permission to film, take photos and/or record anything, NEVER DO SO IN A
BATHROOM OR LOCKERROOM, and never do it of anyone who has not given permission.
INFRACTIONS MAY RESULT IN SEVERE DISCIPLINARY ACTION AND GRADE REDUCTION.
