Please complete this form clearly and return it to the Elections Office, University Offices, Wellington Square, Oxford OX1 2JD, by 4pm on Wednesday 6 May 2015.
Candidates must also be nominated by at least 50 members of Convocation, on an official nomination form, which may be downloaded from the University website ( or obtained from the Elections Office (e-mail: ). Faxed forms or electronic signatures will not be accepted.

Part C: Candidate statement
Each candidate for election, or authorised representative, may submit for publication a written statement, of no more than 500 words, setting out the candidate's reasons for standing. In the event of a contested election, these statements will be published in the University Gazette and on the official University web site.
