Increased Payment for Proactive GP Support to Care Homes Scheme:
In response to feedback from practices participating in the scheme about the level of GP input required, it has been agreed that the payment for the Proactive GP Support to Care Homes will be increased to £250 per patient, per year (previously £200).
The impact of the scheme is already being seen in improved outcomes for patients. According to the latest data from CSU, the scheme has been successful in reducing non-elective admissions (NELs) by 12% in those homes taking part and the trend is continuing downwards. This compares to a 6% rise in admissions in homes that are not taking part.
Currently, 53% of eligible care homes in Oxfordshire are covered under the scheme, and 33 practices are participating. Any practices interested in taking part are welcome to contact Sara Wallcraft () or Jill Gillett () to discuss it further.

Experienced Practice Manager Required for Supporting Vulnerable Practices:
OCCG are looking for experienced practice managers who might be interested in providing support to vulnerable practices. This would be ad hoc work, conducted on either a Tuesday or Thursday, with payment based on a sessional hourly rate of £35 per hour plus mileage. The task involves taking part in an initial diagnostic visit at a practice to understand its challenges and to identify potential solutions. Further follow-up support may be required depending on the nature of issues facing the practice. The CCG may wish to identify a number of practice managers able to assist with this work as practices may prefer to work with a practice manager with experience of their locality.

Diabetes Foot Care:
Thames Valley Strategic Clinical Network (TVSCN) Diabetic Foot Reference Group issued the Diabetes Foot Care Pathway in October 2015. This pathway provides advice about footcare in diabetes, i.e. explain annual footcheck process, what to look out for, describe what good footcare look like and advice how to carry out foot examination.

Practice nurses and other healthcare professionals involved in diabetes care and footcare can also access a free online resource: Foot Risk Awareness and Management Education (FRAME). The online FRAME training modules explain the purpose of foot screening; demonstrate how to carry out a foot assessment and how to use the online SCI-DC foot risk tool to calculate and record foot risk score. There is an assessment involving case scenarios at the end of the modules which the learner may opt to undertake and which, if passed, gives a certificate of completion.

National Diabetes Prevention Programme (NDP) – Your Help Appreciated:
In 2015, OCCG and Public Health developed a joint bid to Public Health England, expressing interest in being part of the first wave demonstrator sites for the National Diabetes Prevention Programme. Unfortunately, the bid was not successful. However, there may be an opportunity to enter this year, but regardless, PHE will be rolling out a programme featuring primary care development of pre-diabetes patient registers. It is hoped that if Practices are already gathering this information, the process will not be onerous when it is rolled out.
We would be very grateful if, as a GP or Diabetes Specialist, you could help the NDPP Steering Group at OCCG to collate information on whether and how Practices are recording pre-diabetes status of patients (although there is currently no actual read code for this). Domestic Abuse Survey:
Oxfordshire County Council are running a Training and Skills Survey for the Oxfordshire Domestic Abuse Strategic Review 2016 hereTraining and Skills Survey for the Oxfordshire Domestic Abuse Strategic Review 2016
The results will help them to get a good view on training requirements in order to plan appropriately for the future. There is no need to register and the survey should only take five minutes to complete and will close on 31 May 2016.

Children with Disabilities Consultation:
Oxfordshire County Council will be recommissioning services for disabled children from April 2017 to include community short breaks, holiday play schemes, after-school activities, youth groups, short break overnights (respite) at Viking House and Sycamore and the Children’s Home at Summerfield.
