FY 2010 Community Health Initiatives Funding Report

I: Intent

In keeping with our mission to bring resources to public health on the North Shore, in FY 2011 the North Shore Community Health Network allocated $98,835.00 to community-based organizations on the North Shore to support primary, preventive health programs and services. Funding is provided in partnership with Northeast Health Systems, North Shore Medical Center, and Lahey Clinic via a state regulatory process to support Community Health Initiatives. The primary goals and intended outcomes for FY 2011 fundingreflect local health priorities and are as follows:

Health Disparities: To ensure that North Shore residents have equitable health care access, treatment and outcomes.

  • Increase the number of North Shore residents utilizing health services and programs.
  • Increase the number of North Shore residents who achieve positive health outcomes.

Chronic Disease: To ensure that North Shore residents have equitable health care access, treatment and outcomes.

  • Increase the number of North Shore residents utilizing health services and programs.
  • Increase the number of North Shore residents who achieve positive health outcomes.

Wellness: To improve community environments through policy and researched-based approaches to physical activity and good nutrition.

  • Decrease rates of unhealthy behaviors such as overweight, lack of physical activity, and consumption of adequate fruits and vegetables.
  • Improve individual functioning through wellness programs and service.

II: Process Overview

As part of the Network’s annual planning process our governing body reviews health data, receives input from other Network members and community partners and then sets: 1) our strategic direction, 2) our funding themes, and 3) our annual operating budget.

In FY 2011, the Network budgeted $100,000to support a competitive grant process that would fund local health priorities. New in this round of funding is a commitment to a 2-year funding cycle (10K per year) pending reports and progress from grantees at the end of the first year of funding. An additional $25k will be made available later this year to fund senior health initiatives, and $50k has been allocated to support a regional public health project managed by the Danvers Health Department.

Our competitive funding opportunity was announced on December 1, 2010 in a variety of ways: web site posting, emails to our 500+ database, via other community partners (United Way, Essex County Community Foundation), at Network meetings, and by word of mouth. A bidders conference was held in mid December to answer questions about the application and funding intent. Twenty-three organizations attended. Thirty-three (33) Letters of Intent were received by December 30, but only 26 applications were submittedfor review (compared to 43 LOI’s and 36 applications from 2010). There were 8 applications reviewed under Chronic Disease, 8 under Disparities, and 10 under the Wellness category. On March 2, a team of nine volunteers, 3 per category, met to review, score, and make funding recommendations. Volunteers were screened for conflict of interest, and represented a diverse group of professionals including a health department, a prevention specialist from outside our funding region, a youth serving agency,a health care plan, a public health nurse, a community foundation, a food pantry, and legal services agency.

III: Funding Summary

Eleven (11) out of twenty-six applying organizations (42%) received funding (compared to 63% of applying organizations last year). In 2010 we had $150,000 compared to 100,000 in FY 2011 to fund competitive grants.

Of those funded, nine (9) received full funding, according to the amount requested on their application, and two (2) received partial funding.The nine fully funded represented the top 3 highest grants scores within their category and the top three ranking positions in terms of review recommendations to fund.
