Connaissances en informatique / Computer skills:

  • Needs to have basic computer skills related to word processing, spreadsheets and power point.
  • Familiar with pharmacy and medical operations programs and drug information software.
  • Willing to receive basic training in using Corel Paradox or Microsoft Access.

  • Significant experience working in an interdisciplinary team environment.
  • Evidence of meeting the pharmacist practice standards used by the Ontario College of Pharmacists, the Faculty of Pharmacy at the University of Toronto and the National Association of Pharmacy Regulating Authorities.
  • Evidence of Practice experience in a primary care environment.
  • Demonstrated expertise in practicing Pharmaceutical Care, including activities such as conducting medication histories, identifying medication-related problems, generating care plans, monitoring care plans, documenting in the medical chart and communicating or collaborating with the interdisciplinary care team as required.
  • Experience in training pharmacists / pharmacist students in the provision of pharmaceutical care.
  • Able to demonstrate a clear vision of how pharmacists can practice in a family medicine group practice.

4.ÉDUCATION ET QUALIFICATIONS PARTICULIÈRES REQUISES (minimum requis)

EDUCATION AND SPECIFIC JOB REQUIREMENTS (minimum required)

  • Bachelor’s degree in pharmacy is required
  • Registered in Part A of the OntarioCollege of Pharmacists is required.
  • Community or Hospital Pharmacy residency or equivalent clinical experience in either setting is required.
  • Master in Clinical Pharmacy or Pharm D. is an asset.

At least 3 years of practice experience is required to demonstrate competencies listed above.

6.SOMMAIRE ET RESPONSABILITÉ GÉNÉRALE / SUMMARY AND GENERAL ACCOUNTABILITY

The role of the pharmacist mentor in this research / demonstration project is to provide clinical knowledge and skill support to the pharmacist integrated in the family medicine group practice. The mentor will ensure the pharmacist is progressing within the practice through motivation, assisting with the development of the pharmacist’s goals and objectives and providing the necessary supportas required. The mentor will ensure the pharmacist is able to meet the documentation requirements of both the family medicine group practice and to satisfy the research objectives described in the project. The mentor will meet with the pharmacist on a regular biweekly basis and be available remotely as required.

  1. PRINCIPALES TÂCHES ET RESPONSABILITÉS / MAIN DUTIES AND RESPONSIBILITIES
  • Meet with pharmacist– The mentor will meet with the pharmacist on a regular basis at the practice site to ensure the pharmacist is able to practice effectively within the unit, has the necessary skills and knowledge and is documenting according to the standards of the group practice and to satisfy the research project needs. In special circumstances some meetings held via other methods (e.g. telephone) will be acceptable.
  • Remote availability to the pharmacist– The mentor must be available remotely (e.g. phone or email) to assist the pharmacist on an ad hoc basis. They will provide support for individual patients or circumstances that require immediate action. The time taken to provide this ad hoc support will be monitored and if deemed excessive by the research team appropriate steps will be taken to reduce the amount to an acceptable level.
  • Collaboration with research support member– The mentor will collaborate with the designated research support person to provide all of the necessary support required by the pharmacist working within the family medicine group practice site.
  • Collaboration with other mentors– The mentor will periodically meet, via teleconference or another suitable media, with other mentors to share information about challenges and successes with the pharmacist integration process and learning needs.
  • Research project documentation – Appropriate documentation will be required to satisfy the research project objectives.
  1. JOB SPECIFICATIONS
  • Physically will be located at their current work site which must be located near the family medicine group practice sites.
  • Hours of work will generally correspond to the family medicine group practice operations but may vary based on the needs of the patients and the needs of the research project. It is anticipated that that the workload will be higher at the beginning of the project (eg. 10- 15 hours for the first 2 months) and be reduced over the remaining time of the project.
  • Work on a term basis for 1 year to start as arranged with the research project team.
  • A contract will be negotiated with the incumbent to specify all salary and benefit details.
