Position Title:Licensed Practical Nurse (LPN)
Reports To:Center Directoror Director of Behavioral Health and Substance Abuse Services
Classification:Hourly
Attendance at HIPAA and compliance educational programs is a requirement of employment.
SUMMARY:
The Licensed Practical Nurse (LPN) assists the Medical Provider or Registered Nurse (RN) in the care and treatment of Health Partners of Western Ohio’s patients. The nurse performs nursing services utilizing the nursing process. The LPN shall perform a variety of complex nursing tasks and related duties as required in a patient centered medical home setting.
DUTIES AND RESPONSIBILITIES:
1.Embraces an active and healthy lifestyle including smoke-free living, stress reduction, exercise, work/life balance and healthy dietary choices.
2.Participates in and promotes health and wellness activities within the organization.
3.Supports a patient-centered healthcare home where the patient-provider relationship is encouraged, with an emphasis on coordination of care and the establishment of a long-term healing relationship.
4.Supports open patient access to and coordination of care that is supported by information technology.
5.Promotes effective and appropriate use of integrated health services at HPWO as part of a “Patient Centered Medical Home” (PCMH) model of care.
6.Performs client assessment utilizing the nursing process.
7.Documents all nursing care provided according to policy.
8.Formulates plan of care for clients under direction from providers.
9.Obtains referrals from other physicians and specialists.
10.Administers medications as prescribed by the provider.
11.Follows approved protocols/standing orders for client care.
12.Follows universal precautions.
13.Performs treatments, procedures as prescribed by the provider.
14.Performs technical skills.
15.Responds to client/family/caregiver needs.
16.Responds to crisis situations according to policy.
17.Utilizes client education for emotional and physical health maintenance.
18.Utilizes supplies in a cost-effective manner.
19.Performs clinic and outreach related tasks.
20.Utilizes on-going assessment skills.
21.Participates in Quality Improvement activities.
22.Demonstrates effective communication skills.
23.Establishes and maintains professional working relationship with other members of the health care team.
24.Participates in planning and problem solving.
25.Participates in continuing education and professional growth.
26.Attends staff meetings.
27.Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures.
28.Assists with seasonal programs as needed.
29.Assists other team members.
30.Performs nursing visits.
31.Completes patient health screenings including hearing, vision, and all CLIA waived testing.
32.Maintains stocks of medicines and medical supplies as necessary.
33.Educates and advises patients on specified medical issues within established parameters.
34.Performs patient education and discharge instructions.
35.Completes referrals and schedules tests for patients.
36.Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
37.Performs other duties as assigned.
WORKING CONDITIONS AND PHYSICAL EFFORT:
Prolonged sitting or standing may be required. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with duties in a clinic office setting will be performed on a repetitive basis. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals, fax machines, and copiers is required. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.While performing the duties of this job, the employee is frequently exposed to communicable diseases, toxic substances, medicinal preparations and other conditions common to a clinic environment. The noise level in the work environment is usually moderate. Working under stressful conditions as well as irregular hours may be required.
QUALIFICATIONS:
- Graduate of a school of nursing accredited by National League of Nursing.
- Current license to practice as a Licensed Practical Nurse in the State of Ohio.
- One to three years full-time experience in nursing practice is preferred.
- Ability to work as a team member.
- Computer experience desirable.
- Ability to work with a culturally diverse group of people.
- Must be willing to work flexible hours as required by position.
