Job Title:Registered Nurse(RN) July 2016

Position Summary

The Registered Nurse will use the nursing process to perform skilled nursing functions in accordance with federal and state guidelines. This position collaborates with multi-disciplinary team members to provide quality, affordable, patient centered healthcare. The Registered Nurse reports to the site Practice Manager.

Primary Responsibilities/Duties

  • Prepares patients forappointment by capturing important medical data. Reviews the patient’s medical record and checks it for completeness to be sure prior test results and/or consultation reports are in. Uses interpersonal communication skills to accurately document the patient’s signs, symptoms & processes that denote the individual’s health needs or potential health problems to give the provider sufficient preliminary information to address the patient’s condition. Follows standing orders & protocols to prepare patients with specific symptoms/diagnoses for appointment. Helps the patient to feel as comfortable as possible by providing emotional support & reassurance.
  • Promotes patient’s independence by establishing patient care goals; teaching patient, friends and family to understand condition, medications and self-care skills; answering questions.
  • Supports clinician and patient by following up on patient care as needed. Calls patient to obtain progress related to treatment plan or provide test results. Documents in the patient’s medical chart in a timely manner to ensure good continuity of care. Refills medications per standing order and completes prior authorization forms as needed.
  • Carries out laboratory functionseffectively and in compliance with the Clinical Laboratory Improvement Act (CLIA) regulations. Maintains OSHA regulations and Universal Precautions while performing laboratory functions. Obtains and prepares specimens for analysis by off-site reference laboratories.
  • Prepares examination/treatment rooms and equipment in a way that facilitates the efficient and safe assessment and treatment of health center patients. Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer’s instructions; calling for repairs. Maintains medical supply inventory by checking stock to determine inventory level; anticipating needed supplies, placing orders for supplies.
  • Ensures that procedures/treatments that are conducted during the visit are charged appropriately in the electronic medical record. Facilitates provider efficiency by managing the clinician’s patient schedule and supporting clinician in such a way that the clinician can focus on direct patient care. Supports fellow staff members by providing coverage during absences or assisting on busy days.
  • Utilizes tools to support patient care such as Immpact, HealthInfoNet, Prescription Monitoring Program, etc.
  • Pursues appropriate training/education to improve areas of job performance. Shares relevant information obtained at educational programs with other staff during health center staff meetings.
  • Participates in quality improvement activities. Understands and utilizes population health management and quality improvement reports.
  • Demonstrates leadership skills by assuming responsibility for higher level activities above “core” job functions. Serves as CHC representative on organizational committees (CLIA and Clinical Support) and assumes responsibility for following up on assignments generated at organizational meetings.
  • Performs all other duties as assigned to meet the ongoing needs of the organization.

Qualifications (Education/Experience)

  • Graduate of an accredited school of Professional Nursing and holds a current Maine RN license.
  • CPR Certification required.
  • Must have solid computer skills.
  • Must have strong listening and communication skills as well as being compassionate and sympathetic to patient’s needs.
