INSTRUCTIONS FOR COMPLETING A CUSTOMER SERVICE REQUEST (CSR)

Client#:Type in your two-character client code in the first box. In the second box, type in the internal tracking number you have assigned to this request.

Date Submitted:The date you are sending the request to QSS.

Date Required:The date you need to have either: the problem resolved, the inquiry answered, or the quoted project completed. Date required is not appropriate for suggestions.

Type of Request:Mark “Problem” if there is an error in how a program functions or in the documentation. Mark “Inquiry” if you have a question on how a program or process works. Mark “Quote” when you are requesting a new program, such as a report, or if you are requesting a modification to an existing program. Mark “Suggestion” when you want to submit ideas for consideration. All suggestions are considered and, when appropriate, are included in future enhancements, without charge to the client(s) submitting the suggestions.

Client Name:Type in the name of your district or county office.

Submitted by:The name of the person submitting the request.

Phone:The phone number of the person to contact if there are questions about the request.

Program/Process:The name of the program the request pertains to. If you are not sure which program, put down the process involved, such as payroll update. Please be as specific as possible. Do not just say “running payroll”, but the exact step that

you have a question or problem with.

Subsystem:The program’s subsystem. For example, PAY, GLD, STUDENT GRADING, MQP, etc.

Version:The version you are currently running of the program. If this information is not displayed on the screen when you run the program, it can be obtained by running GETVER.UTIL.PSIPROG.

Compile date:The compile date of the program, also determined by running GETVER.UTIL.PSIPROG.

Description of Request:Please be very specific when filling out this section. Give as much detail as possible. Include copies of reports, copies of screens, $STDLISTS, etc. Requests that do not have adequate documentation will be returned to the sender.

Keep a copy of the completed CSR for you files. We suggest you use the internal tracking number as a method of monitoring the CSR’s you submit. QSS will assign its own number to each request received. When QSS has completed the appropriate action, a copy of the CSR will be returned to you with the response box filled in. If this is a request for a quote, a Quote Acceptance Form will also be sent to you for you approval. No work will be done on a quote until this form is returned with the authorizing signature, date, and purchase order number.
