RETURNING STUDENTS:
I have examined the expiration date of my I-20 or DS-2019
I have examined the expiration date of my passport
I have examined my student account to ensure that all university fees are assessed correctly (including insurance)
I have filed or will file an insurance waiver, if needed, prior to the 5th class day of the next semester
I have submitted the “Under-enrollment Form” (available on the OISS website) because I’m graduating next semester and I will not be enrolled full-time
I have completed the OISS “Change of Address” form (available on the OISS website) within 10 days of moving
I have updated my mailing address with UNO via webstar
PRACTICAL TRAINING (CURRICULAR OR OPTIONAL):
I plan to do an internship in the upcoming semester and have applied or will apply for Curricular Practical Training (CPT) before I begin working
I am graduating this semester and have applied for Optional Practical Training (OPT). I mailed the application to USCIS so that they would receive it before the last date of the semester or no later than2 weeks before the end of the semester
TRAVELING:
I have obtained the necessary signatures and supporting documents to return to the U.S.
At least 2 weeks before traveling out of the U.S., I have brought the following to OISS:
  • Pink Student Request form filled out correctly
  • I-20/DS-2019
  • Passport & I-94 card
