PRIZE DRAW TERMS AND CONDITIONS:

1. The prize draw opens at 12noon on Friday 9th May 2014, and closes at 12noon, Friday 6th June 2014.

2. Entries received after the closing date will be deemed ineligible.

3. Entry is by completing the online survey accessed via the free Connecting Memory app (Download app and click Info) including your name, email and phone number.

4. Only one entry per person will be accepted.

5. People under age 18 are not eligible to enter.

6. Prizes are as follows: First Prize – Two (2) tickets to a Geelong Performing Arts Centre performance of your choice in 2014, valued at approx $100; Second Prize - Two (2) tickets to a Potato Shed Multi Arts Centre performance of your choice in 2014, valued at approx $70; Third Prize - An Event Bag of surprises, including an M2M Contemporary Songline CD/DVD pack, M~M2014 T-Shirt and a free Family Pass to the National Wool Museum, valued at approx $60; Fourth Prize - One of two iTunes vouchers, each valued at $50, Total prize pool is valued at approx $330.

7. Prizes are not transferable or redeemable for cash.

8. Ticket prizes are subject to availability at the time of booking.

9. Prize winners will be drawn at 12pm on Monday 9th June 2014 at the National Wool Museum.

10. The first five fully completed entries drawn will be the winners. The decision is final and no correspondence will be entered into.

11. The winners will be notified by email, or phone if no email is provided.

12. The winners will be advertised on the M~M, Events Geelong and Geelong Mayor Facebook pages by 9am on Tuesday 17th June 2014.

13. Employees of Deakin University and the City of Greater Geelong are eligible for thisprize draw.

You are invited to take part in this research project. This Plain Language Statement contains detailed information about the research project. Its purpose is to explain to you clearly all the procedures involved in the project so that you can make an informed decision whether you are going to participate.

You are asked to complete a survey. Consent is implied by completing the survey on the App.

2. Purpose and Background

The purpose of this project is to evaluate the effectiveness of Mouth to Mountain project delivered by the City of Greater Geelong More specifically the project aims to investigate your experience, the degree to which you have established a connection with Geelong and the nature of your engagement.

You are invited to participate in this research project because you are a participant in the project.

The results of this research may be used to help researchers (Prof Ruth Rentschler, Dr. Tabitha White, Deakin University) to evaluate the effectiveness of the project for the betterment of Geelong as it transitions from a manufacturing base to service industries and a knowledge society.

3. Funding

This research is totally funded by Deakin University

4. Procedures

The project entails you completing the survey on the App, which takes about 15 minutes. You information will be kept confidential by Deakin University. No details will identify you as the respondent. While the App has been developed by the City of Greater Geelong, the data is downloaded directly onto a spreadsheet that is provided directly to Deakin University researchers. City of Greater Geelong does not have access to the data nor will they use the data.

Your involvement in the project is voluntary. If youdon't want to participate,then please do not send us your answers.

5. Possible Benefits

The expected benefits for you are that you will be able to provide your experiences as a participant on the transition of the City of Greater Geelong in its development as the economy transitions from one based on manufacturing to one based on service and the knowledge economy.

6. Possible Risks

If you are uncomfortable at any stage of the survey, simply exit the App and your information will not be used.

7. Privacy, Confidentiality and Disclosure of Information

The questionnaire information that you provide is confidential, and no information that could lead to the identification of any individual will be disclosed in reports or publications from the project, or to any other party. In any publication, collective information will be provided so that you cannot be identified.

All data (electronic) will be stored on computers with passwords at Deakin University. The data will not be available, accessible or used by the City of Greater Geelong for any purposes. Data on the App is downloaded directly onto spreadsheets for use by Deakin University researchers. No one else except the researchers will have access to the data. The data will be stored for a period of 5 years as per the requirements of Deakin University and after that period it will securely be destroyed.

8. Results of Project

Aggregated results from the project will be communicated to the wider academic field through presentations in national and international conferences. Aggregated results will be provided to the City of Greater Geelong for the purposes of assessing the nature of the experience that you engaged in.

9. Participation is Voluntary

Participation in this research project is voluntary.If you do not wish to take part you are not obliged to.

Your decision whether to take part or not to take part, or to take part and then withdraw, will not affect your relationship with Deakin University or the City of Greater Geelong.

10. Ethical Guidelines

This project will be carried out according to theNational Statement on Ethical Conduct in Human Research(2007) produced by the National Health and Medical Research Council of Australia. This statement has been developed to protect the interests of people who agree to participate in human research studies.

The ethics aspects of this research project have been approved by the Human Research Ethics Committee of Deakin University.
