SUBJECT: Revisions to Standard of Practice (SOP) 2.6.5 and Associated Forms based on Regulatory Changes to 922 KAR 1:010 Independent Adoptions
The purpose of this memorandum is to inform staff of the regulatory changes adopted November 16, 2005 to 922 KAR 1:010 Independent Adoptions, formerly known as Application for Permission to Place or Receive a Child, and subsequent revisions to SOP 2.6.5 Independent Adoption Application, formerly known as Application for Permission to Place or Receive a Child. These changes include:
· The nonrefundable application fee increasing from $150 dollars to $200 dollars;
· Applications for an unborn child will no longer be accepted by the Cabinet;
· Independent Adoption Home Study replaces the term/phrase placement investigation and
· Revised / new forms incorporated by regulation, which include:
o Revised DPP-105, Medical Information on Child’s Birth;
o New DPP-108A Health Information Required for Proposed Adoptive Parent;
o Revised DPP-187, Independent Adoption Application, formerly known as DSS-187 Independent Adoption Application for Permission to Receive or Place a Child;
o Revised DPP-190, Information to be Obtained from Proposed Adoptive Parent(s), formerly known as DSS-190 Information to be Obtained form Receiving Parent(s);
o Revised DPP-190A, Supplemental Information to be Obtained from the Proposed Adoptive Parent(s) formerly known as DSS-190A Supplemental Information to be Obtained from the Receiving Parents
o Revised DPP-191 Information to be Obtained from Placing Parent(s); formerly known as DSS-191;
o New DPP-191A Information to be Obtained from Placing Parent for Independent Adoptions;
o Revised DPP-192 Biological Parent Consent Form;
o Revised DPP-197 Adult Child Interview; and
o Revised DPP-199 Independent Adoption Home Study Outline, formerly known as the Independent Adoption Placement Investigation Report.
There will not be a field review of the SOP revisions due to the regulatory requirements necessitating these revisions. The regulatory changes pertain to procedures that are performed primarily by Central Office staff, however field staff need to be aware of the changes so they can respond to client inquiries and provide the current revised / new forms. Revisions to SOP 2.6.5 Independent Adoption Application are effective December 1, 2005 and supersede all previously issued Standards of Practice or policy regarding this topic. Revisions to SOPs are indicated by red print.
