Post: consultant psychiatrist in eating disorders (6 PA’s)

Applications are invited for this post which provides medical input to the NHS Greater Glasgow & Clyde Adult Eating Disorder Service (AEDS). The service currently consists of a 4 inpatient beds at MacKinnon House, Stobhill Hospital and a community team providing intensive out-patient care. The service is currently under review and may be extended to provide a day hospital service. The post holder would be expected to take an active role in development of the service and also contribute to the safe management of eating disorder patients within Acute settings.

The post holder will be expected, following completion of service redesign process, to have psychiatric responsibility for the care of those patients with a severe primary eating disorder diagnosis across NHS GG&C. Patients requiring in-patient admission will be admitted to the 4-bedded in-patient unit under the care of the post-holder. The post-holder will also act in a consultative capacity to locality Consultants caring for sector patients with eating disorders with the support of the AEDS community team.

This is a service which adds to the excellent range of services across NHS Greater Glasgow & Clyde, which has well-developed integrated CMHTs and Crisis Teams, as well as specialist services including Perinatal Psychiatry, Psychotherapy and First Episode Interventions. There are also strong links with the Department of Psychological Medicine at the University of Glasgow, so that there are excellent opportunities for research, and it is expected that the postholder would be granted the status of Honorary Lecturer.

Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in General Adult Psychiatry or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

Job Description

CONSULTANT IN EATING DISORDERS – NHS GREATER GLASGOW & CLYDE

  1. Applications are invited for this 6-session post based at Stobhill Hospital / Florence Street Resource Centre, Glasgow. The post-holder will provide medical leadership and care for AEDS (Adult Eating Disorder Service) community patients and inpatient beds based at MacKinnon House, Stobhill Hospital in North Glasgow. The post-holder will be supported by a Team Secretary at base and a Medical Secretary at Stobhill Hospital.
  1. Service Provision

The service is currently under review and the postholder will be expected to contribute to the further development of the service.

AEDS has been providing community care for patients with eating disorders throughout Greater Glasgow since January 2008. It aims to treat patients primarily in the community, and to facilitate discharge from hospital, where a period of in-patient treatment has been necessary. The AEDS community team have previously managed patients in partnership with the CMHTs, however following a service review it is expected that the medical role will be extended to provide medical care to those patients with a severe primary eating disorder diagnosis across NHS GG&C with the support of the AEDS community team.Referrals to AEDS community or inpatient service are routed through the CMHTs.

The post-holder will provide medical leadership to AEDS across inpatient and community sites. The expectation would be that he/she will also have an advisory role for CMHT Consultants and for GPs across NHS GG&C. He/she will further develop services for eating disorder patients within Acute settings as medical lead for the MARSIPAN group.

Admissions to the inpatient unit are planned, and the AEDS community team continue to provide input throughout the patient’s stay on the ward. There are 4 inpatient beds based on a general adult ward at MacKinnon House in Stobhill Hospital, where the general staff also have a high level of expertise in the treatment of eating disorders.

Some eating disorder patients may be admitted to their locality wards when appropriate, and in discussion with the post holder who would be expected to work jointly with the CMHT Consultant to manage care during the inpatient stay.

Junior doctor cover for the inpatient service will be provided by the Duty Doctor at Mackinnon House, however junior doctors may spend sessions in the eating disorder service by agreement. It is expected that the post-holder should apply to become an ST4-6 Trainer.

There is a currently a Day patient programme based at the Florence Street Resource Centre which runs for 2 days a week offering a service both to patients in the community and inpatient beds. AEDS inpatients receive input from the OT department and the AEDS day patient programme throughout their admission, when physically able.

However, as part of the AEDS service review, a Day Hospital model is actively being developed and it is expected that the medical input will be defined within this development plan and be included within current resource . A Medical Officer provides physical health care input to the community service.

The AEDS community team currently comprises of a Consultant Clinical Psychologist, who is the designated Clinical Lead for the service, 1 wte Band 7 Team Co-ordinator, 2.6 wte Clinical Psychologists, 0.3 Medical Practitioner, 1.4 wte Dietitians, 1wte Occupational Therapist, 7.4 wte Band 6 Nurses, an Assistant Psychologist and a Team Secretary.

  1. Governance Arrangements

AEDS is managed by Katrina Phillips, Head of Adult Services for North East Glasgow HSCP and the Clinical Director is Dr Ruth Ward.

Mental Health Services are divided into four sectors, South, West, North & East and Clyde and the Lead Associate Medical Director is Dr Michael Smith. The Chief Officer is David Williams.

  1. Teaching and Administration

The post-holder will undertake the administrative duties associated with the care of patients and the proper function of AEDS. He or she will be expected to participate in the broader aspect of administration and management within the North East Glasgow Division of Psychiatry.

It is expected that the successful candidate will take part in Continuing Professional Development, and participation in audit or research is encouraged. The postholder will be actively involved in teaching medical students. It is anticipated, in recognition of this, that Glasgow University will grant the status of Honorary Clinical Senior Lecturer in Psychological Medicine. In addition, the post-holder will be expected to participate in the postgraduate teaching of medical staff and staff of other disciplines.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

QUALITIES REQUIRED

Essential

  • MRCPsych or equivalent
  • An ability to interact professionally with colleagues, patients and carers
  • A commitment to and working knowledge of multi-disciplinary working
  • A commitment to ensuring the delivery of high quality mental health services at a local (CMHT) and possibly area level
  • Ability to work collaboratively within a multidisciplinary team
  • The ability to work with, and integrate psychological formulations alongside psychiatric diagnoses
  • Wide experience in the provision of adult mental health services
  • Experience of treating patients with eating disorders
  • Knowledge of current issues in General Adult psychiatry
  • Experience of working in a community mental health setting
  • Section 22 approval or be eligible for section 22 training and approval.
