do hereby grant permission for the aforesaid participant to participate in:

Youth Ministry’s “Bonfire Night” at Saint Albert the Great on Wednesday, August 2nd, 2017. The fire will begin at 8:30PM, with “doors” opening at 8pm. If permitting, there will be a water balloon fight before/during the bonfire. If engaging in the water balloon fight, please BRING A BAG of water balloons as your own ammo. There is no limit on how many water balloons you can bring. The bonfire is to conclude at 10PM. Members will be sitting around the bonfire, making s’mores,playing games and discussing various topics. Please meet at the baseball diamond nearest to the parking lot.

*** We will be cancelling the event in the case of rain or muddy field conditions. Please check our website on WEDNESDAY for cancellation details.***

I/we agree by my/our mutual signature(s) to release, indemnify and hold harmless “Bonfire Night,” the Youth & Young Adult Ministry and CYO Office, the Catholic Charities Service Corporation, Catholic Charities, and its affiliates, the Roman Catholic Diocese of Cleveland Churches or Parishes, Saint Albert the Great, and any and all supervisors, volunteers, organizers or sponsors thereof, and from any and all liability for injury, medical fees, hospital bills, or doctor bills of aforesaid participant. I/we waive all claims of any kind against any or all of the organizations or persons hereinabove enumerated, including any all claims against persons transporting aforesaid participant to or from activities hereinabove named.

I/we hereby give consent to photograph or videotape aforesaid participant and without limitation to use such photographs or videotapes and or stories in connection with this event for the sole purpose of any work of Saint Albert the Great, the Youth and Young Adult Ministry and CYO Office and I do hereby release Saint Albert the Great, the Youth and Young Adult Ministry and CYO Office from any claims whatsoever which may arise in said regard.
