LICENSURE INFORMATION AND APPLICATION INSTRUCTIONS
Licensure and regulation of Speech-Language Pathology and Audiology in the State of West Virginia is governed by Chapter Thirty of the West Virginia State Code, One Thousand Nine Hundred Thirty-One, as amended by Article Thirty-Two. The effective date of this Article is June 5, 1992. A copy of this law can be obtained by calling 1-304-558-6000. If you would like Rules Governing the Licensure of Speech-Language Pathology and Audiology call the same number and ask for Title 29, Series 1.
Licensure and regulation of Speech-Language Pathology and Audiology in the State of West Virginia is governed by Chapter 30-32 of the West Virginia State Code. The effective date of this Article is June 5, 1992. The law and rules governing the practice of Speech-Language Pathology and Audiology are available on our website. It is the licensees responsibility to know and understand the law that regulates his or her profession.

THE FOLLOWING GUIDELINES WILL HELP YOU DETERMINE YOUR LICENSURE ELIGIBILITY
APPLICATION FOR LICENSURE-- All questions must show a response. Please indicate N/A if the question is "not applicable" to you. Incomplete applications will not be processed. Your signature must be notarized.
EXEMPTIONS--The following individuals are not mandated to be licensed under this article:
A. Physicians and surgeons licensed in West Virginia.
B. Individuals while engaging in their professions providing hearing testing or balance system testing under the direct supervision of a licensed physician or surgeon, so long as such
individuals do not hold themselves out to the public as possessing a license or using a title as set forth in the Article.
C. Individuals employed or contracted with the West Virginia Board of Education, a County Board of Education or a Regional Education Service Agency.
D. Individuals duly credentialed as a Teacher of the Deaf in West Virginia.
E. Individuals pursuing a course of study leading to a degree in Speech-Language Pathology or Audiology at a college or university.
F. Non-residents engaging in the practice of Speech-Language Pathology or Audiology in West Virginia if their activities do not exceed five days in any calendar year.
Please refer to Chapter 30-32-5 of the State Law for further exemption details or write to contact the Board office if you have further additional questions.
To register your exemption status with the Board, check "Exemption Request" and submit your completed application. No fee is required to register under the exempt category. Remember, even though your primary employment places you in an exempt category, if you practice Speech-Language Pathology or Audiology under any non-exempt circumstances, you must be licensed.

QUALIFICATIONS FOR LICENSURE--To be eligible for licensure as a Speech-Language Pathologist or Audiologist, you shall:
A. Possess at least a Master's degree or equivalent in Speech-Language Pathology or Audiology from an educational institution approved by the Board;
B. Pass the National Examination in Speech-Language Pathology or Audiology which is approved by the American Speech-Language-Hearing Association;
C. *Complete supervised clinical practicum experiences from an educational institution or its cooperating programs; and
D. *Complete a clinical fellowship year/postgraduate professional experience.(PPE)
*Any person who has completed a clinical doctorate in audiology (Au.D) from an accredited institution fulfills the requirement for the Postgraduate Professional Experience.
WAIVER OF REQUIREMENTS
RECIPROCITY -- If you are applying for a West Virginia License on the basis of reciprocity of license as a Speech-Language Pathologist or Audiologist in another state, you must present proof of current licensure in a state that has standards that are at least equivalent to those of this state. A Letter of Good Standing from the licensing board where you are currently licensed is required. Please have it mailed directly to our Board Office.
If you have a current license from another state with equivalent standards with West Virginia or hold the Certificate of Clinical Competence from the American Speech-Language-Hearing Association you may practice Speech-Language Pathology or Audiology in this state, pending Board disposition of your application provided you;
A. Are practicing in the area, Speech-Language Pathology or Audiology, in which your license or Certificate of Clinical Competence was granted; and
B. Have filed an application with the Board and paid the appropriate fee.
VERIFICATIONS:
CERTIFICATE OF CLINICAL COMPETENCE -- To verify your current Certificate of Clinical Competence, you must submit a copy of your American Speech-Language-Hearing Association card listing you as a current "Certified Member" or "Certified Non-member". If you do not have your ASHA card, you should contact ASHA and request that they submit a verification statement directly to the Board.
NATIONAL EXAMINATION -- A passing score on the "National Examination in Speech-Language Pathology" or the "National Examination in Audiology" administered by the Educational Testing Service of Princeton, N. J., must be verified by the Educational Testing Service. Verification in this category is required only if you have not received the Certificate of Clinical Competence in either Speech-Language Pathology or Audiology.
PROVISIONAL LICENSE -- The purpose of a provisional license is to permit an individual to practice Speech-Language Pathology or Audiology while completing the required postgraduate experience/clinical fellowship year. (CFY) The provisional license is issued for a period of one year and may be renewed for one additional year. Individuals issued a provisional license are authorized to practice only while working under the direct supervision of a person fully licensed by this state. Except for the postgraduate professional experience, applicants must meet the academic, practicum and examination requirements.
SPEECH-LANGUAGE PATHOLOGY AND AUDIOLOGY ASSISTANTS -- Licensed Speech-Language Pathologists and Audiologists who supervise Speech-Language Pathology and Audiology Assistants must register with the Board the name of each assistant working under their supervision. A $50.00 registration fee applies for the Initial Registration of assistants. Renewal registration for Assistants is required before the last day of the twelfth (12th) month of each year. Please contact the Board for assistant registration forms and guidelines. The Assistant registration form and log sheet can be downloaded from our website under the “Forms” link. If you have questions, please contact the Board Office for assistance.
When all the credentialing and fees are received and approved, your assistant registration will be processed In a timely manner. If we need additional information, you will be contacted.

Provisional License – 1 year $ 50.00
Beginning July 1, 2010 2012 through December 31, 2010 2012 (the last quarter of the 2-year licensing period) rates will be lowered for INITIAL FEES INITIAL FEES for Professional Licenses are pro-rated as follows:
Initial application fee for Professional (Non-Refundable) $ 75.00

Initial Dual license fee for Speech Pathology and Audiology $ 150.00
RENEWAL FEES:

and Audiology (2 yr)
3. Renewal late fee (application post marked after December 31 $ 75.00
