INTRODUCTION
Welcome to Volusia Health Network. It is through your participation that we are able to achieve our mission of providing a managed health care system that offers cost effective, quality services that promote, enhance and maintain health in our community.
Your employer has selected Volusia Health Network to help control health care costs including your out-of-pocket expenses. Through Volusia Health Network you have access to the highest quality physicians, hospitals and other facilities that have agreed to provide services at a discounted rate.
Member-Patient Responsibilities
Please note it is the responsibility of the Employee or Covered Person to make certain that a Volusia Health Network Provider provides the services rendered. This includes the following services: radiology, laboratory, hospital, surgery center, physical therapy and other facility/providers. Please don’t hesitate to ask your Physician if the facility they are using is part of Volusia Health Network. It is also your responsibility to make sure your physician has hospital privileges at an in-network Volusia Health Network participating hospital. Volusia Health Network providers are subject to change. Please check with provider when making appointments to ensure that they still participate with VHN. Failure to receive care/services by a VHN provider may result in reduced benefit coverage, denial of benefits, or having a monetary penalty applied. Questions regarding in-network providers can be answered by calling 386/425-4846or 800-741-2198.
Precertification of care is also the responsibility of the Employee or Covered Person. Know your medical plan – All health plans have some services that require approval prior to receiving care. You should check your plan to see which procedures/care require precertification. You will need to work with your provider in obtaining authorization for proposed care. Please let your provider know that the care that you are about to receive requires precertification. Your provider will then need to submit clinical information to the precertification department representing your medical plan. Precertification of scheduled inpatient, outpatient surgery, or other deemed procedures should be precertified at least 5 working days prior to receiving care. The Precert Department may need to communicate several times with your provider to get the information necessary for authorization. There may also be times when a second opinion may be required; therefore the greater the advanced notice the better. Emergency inpatient admissions must be reported within 24 hours of the next business day after an emergency admission. Failure to precertify before services are rendered will result in reduced benefit coverage, denial of benefits, or having a monetary penalty applied.
Also, please note that precertification is not a guarantee of payment only medical necessity; benefit payments are subject to Plan provisions.
Choosing A Provider
How do I choose a provider?
Although you have the option to choose a non-participating provider for care, you are encouraged to choose a participating provider to minimize your out-of-pocket expenses. The difference in cost to you can be substantial. Refer to your employee benefits manual for the explanation of coverage specific to your plan. If you need help in selecting a provider, please contact Volusia Health Network at 386/425-4846 or800/741-2198 for assistance.
At times it may be necessary for a provider/physician to temporarily close his/her practice to new patients. This is done when the physician’s patient loads are too full to provide the highest quality of care. When patient load allows the provider/physician will then take on new patients.

Member ID Card
Do I need to use my member ID card?
In order to expedite processing, we encourage you to carry your health plan’s ID card with you at all times. Present the card whenever you receive medical services, whether from a participating or non-participating provider. The card contains important information including addresses and phone numbers necessary for claims’ payment, hospital admissions and co-payment information.
Physician Office Visits And Co-Payments
How do I use my Volusia Health Network Provider Network?
When you need medical care, simply refer to the attached listing and select a provider. Call the physician's office or hospital you select to arrange for an appointment and identify yourself as a Volusia Health Network member. When you arrive at your appointment, be sure to present your group identification card. Depending on the benefit plan determined by your employer, you may be responsible for an office visit co-pay at the time of service. Co-payment information is generally listed on your member ID card.
Will I have to file a claim?
Most Volusia Health Network providers will automatically file your claim for you. If the provider submits the claim for you, simply pay your part of the bill and for any service or procedures not covered under your benefit plan. There may be some situations where you will need to submit a claim form with corresponding documentation such as receipts.
What must I do in case of a hospital admission?
Before an elective admission to the hospital, you or your physician must call the Utilization Management/Pre-certification number on your ID card. Advanced notice requirements vary according to benefit plans. Call your benefit administrator or your employer’s Personnel Office for information. The utilization review staff will review the treatment plan with your physician to determine medical necessity of your admission. This process is called pre-certification. If your hospital stay is not pre-certified, your plan benefits may be reduced.
What services require prior authorization by the Utilization Management Department?
The procedures that require pre-certification will vary by health plan. All hospital inpatient services must be authorized prior to the date of admission. The Utilization Management Department must becontacted no later than two (2) working days after any emergency admission.
Outpatient Procedures
Some surgical procedures must be performed on an outpatient basis to obtain the maximum coverage. Contact the Utilization Management/Pre-certification number listed on your ID card for information about procedures that require outpatient services. Some health plans require pre-certification of certain outpatient testing. Review the employee benefits manual supplied by your employer for specific utilization review requirements.

DEFINITIONS
PPO:APPO (Preferred Provider Organization) is an affiliation of physicians, hospitals and other healthcare providers, who form a network to assist self-insured businesses in tailoring health plans to meet the unique needs of their employees.
EPO:An EPO (Exclusive Provider Organization) works the same way as a PPO - however, with a greater discount on healthcare services.
Network: Aterm used to describe the entire group of hospitals,
physicians and other medical professionals who have
contracted with the PPO to provide services at a discounted rate.
Participating Another name for a physician, hospital or other medical
Provider:professional/facility who has contracted with Volusia Health Network to provide services at a discounted rate.
ExplanationAn "EOB" is a form sent to you and the provider after each

Urgent CareA facility with on-site physicians, x-ray and lab equipment.
