REGISTRATION INFORMATION – Submission Instructions:

The organizing committee would be very grateful if you would electronically complete:

2.  The Clothing Order Form

3.  One Chaperone or Team Member Participation Form for each attendee

All completed entry forms and fees must be in the hands of the organizing committee no later

than August 31, 2011. Post dated cheques dated later than August 31, 2011 will not be accepted.

Entry Fee: $350.00 per participant or chaperone. This fee includes accommodations, activities and meals from Friday evening to Sunday evening, and shuttle to and from Stage West Hotel from Pearson International Airport (Toronto YYX). Additional money will be required for Monday breakfast.

Number of teams: Each region is entitled to send one C team of 4 members, one A/B team of 3 or 4 members and one Affiliate team of 2 or 3 members. If you would like to send more teams, contact the

Registrar.

Scramble Teams: In the event that you have 1 or 2 members in the A/B or Affiliate levels that are eligible

to compete, they are welcome to apply and be put on a scramble team with a minimum of 3 members.

Chaperones: One chaperone per team is required.

Armbands: Medical armbands must be worn at all times.

Late Entries:

Acceptance of entries received after August 31, 2012 are at the discretion of the organizing committee, but we will make a valid attempt to accommodate. Late entries will be assessed a fee of $50.00 per team.

Clothing:

National Quiz Wear will be available for pre-order. Refer to the order form posted late June with this registration package. Orders and payment for clothing must be received with your entry form, no later than August 31, 2012. Clothing will be available for pick up on Friday evening at the registration desk.

Stage West is an “All Suites” hotel, which provides spacious accommodations, including (for four or five) two queen beds, 2 TVs, and a separate living area with luxury pull-out queen bed. There are complimentary wireless internet access, iron, hair dryer, work desk, in-room safe, coffee machine, microwave and fridge. The hotel boasts a fitness area and saltwater pool with water slides for lots of fun, so bring a bathing suit (towel service at the pool). Airport shuttle service and guest Parking is included.

Hotel Check-in:

Please note the host region will not cover any incidental room charges (phone bills, meals, movies, etc. which are charged to the room). Guests will be asked to provide a credit card at check-in for incidentals.

Occupancy & Extended Stays:

Members will be booked quad occupancy by team with chaperones also lodged in the Suite.

Assuming room availability, chaperones may upgrade to a double room sharing with a second chaperone for an additional $65/each, or a private room for an additional $125 night (single queen suite). Please make a note on the chaperone’s form. Teams or chaperones may extend their stay and be eligible for National Quiz quad room rate of $135/night. Make a note on all participants’ forms.
