Dear East Boys’ soccer candidate,

The East soccer season is rapidly approaching, and I am very excited about our upcoming season. Enjoy your summer tournaments, camps, and plan to arrive in August in your best physical condition.

Physical exams are required for all soccer candidates. Candidates should submit all forms from the physical packet by Monday August 12th. Please make sure everything is filled out and signed. See reverse side of this sheet for more information regarding physicals and deadlines. Physical forms can be downloaded from the Boyertown School District website at:

http://www.boyertownasd.org/Page/1284 or forms are available at each school.

The forms you need to submit are as follows:

1. 6 pages of PIAA form 3. Student responsibility sheet

2. Emergency form 4. Eligibility sheet

Reminder: No athlete may begin practice without a physical card signed and dated after June 1st, 2013 and the beginning of practice.

Be sure to get all of your paper work in before tryouts begin.

Tryouts will begin Monday August 26th, and continue on the first day of school. The tryouts on Monday August 26th will be held at East, from 3-5pm. Be sure to bring all of your equipment and your water with you. We will meet at East in the back parking lot at by the tennis courts before going up to the field. Tryouts will again be held continuing on the first day of school from 3:00-5:00pm.

Note: There will be no tryouts from Friday, August 30th to September 2nd. On Tuesday, September 3rd, tryouts will resume from 3:00-5:00pm and be held after school everyday thereafter unless notified otherwise.

In order to improve your chances of making a squad, I encourage all of you to begin a physical conditioning program this summer. Good conditioning activities are jogging, bicycling, and swimming, etc. These activities are most effective when done 15-30 minutes per day. Try choosing one activity each day to work on different muscle groups. All players should work with the soccer ball at least three to five times a week. The coaching staff hopes you have a restful and enjoyable summer, but come prepared to work.

Physical Information for the School Year 2013-2014

First Day of Practice for Sports (for Senior High):

Fall – August 12th Winter – November 18th Spring - March 3rd

First Day of Practice for Sports (for Junior High):

Fall – August 19th or 26th Winter – November 18th Spring - March 3rd or 10th

First day of school is: Wednesday, August 28, 2013

The PIAA‘s policy is one physical (dated after June 1, 2013) is good for entire school year. (Student may be required to get a clearance (for winter or spring sport) by a physician if injured in a previous sport season. See Re-Certification information).

Due to a change in our policy we will not be offering school sports physicals for the school year 2013-2014. You will need to schedule an appointment with your family doctor or you may go to Premier Urgent Care in Douglassville.

To insure your son/daughter can start sports on the 1st day of practice, we are requesting you send your son/daughter’s physical forms into the Athletic Office at the Boyertown Senior High School by:

DEADLINE FOR PHYSICAL PACKETS

BEFORE June 7: Return forms to the main office at JHE and JHW and senior high turn them into the

Athletic Office.

AFTER June 7: Students going to grades 10 through 12 should return the packets to the

Athletic Office BY WEDNESDAY JULY 31ST.

AFTER June 7: Students going to grades 7 through 9 should submit packets BY MONDAY,
AUGUST 12th.

You can email the completed forms to , fax to 610-473-5444, drop off at the Boyertown Senior High School or mail to Boyertown Senior High School, Athletic Office, 120 North Monroe Street, Boyertown PA 19512.

If you get a physical at your family doctor please make sure that the doctor signs and dates Section 6 (due to doctor’s offices giving out a copy of After Visit Summary).

It is recommended that you make a copy of the physical form prior to submitting it to school.
