JOB TITLE: Community Support Worker

RESPONSIBLE TO:Hospital Services Co-ordinator

HOURS:35per week (variable shift pattern applies)

SALARY: £8.44 per hour

LOCATION: Salford Royal Hospital.

The role includes home visits across Salford(and in some circumstances may operate outside Salford.)

JOB PURPOSE:To provide personalisedguidance for Salford residents aged 55 and over, who have presented at Salford Royal Hospital andbeen deflected or discharged from the Emergency Village, elderly care wards or other areas of the hospital, as determined appropriate by the HospitalServices Manager. To provide support to improve thepatient discharge process and prevent the risk of hospital re-admissions and in some circumstances to escort the patient home or facilitate access to transport to other appointments, to aid recovery. To provide support and facilitate access to other health, social care,community services, and support groups, in order to support the client’s recovery at home,helping them regain independence and improve their overall wellbeing. To developand maintain strong links with other internal and external professionals, agencies and community groups, in order to provide a responsive and effective service that consistently delivers positive outcomes for older people in the Salford area.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1.Work in a person centred way to support individuals, from their hospital attendance and up to 6 weeks after discharge, using assessment and support planning processes,

2.Visit patients at home after discharge (including escorting people home where no other support is available), ensuring the person is warm, safe, has access to meals and the ability to undertake basic activities of daily living and that other emergency support networks are engaged where this is not possible.

3.Carry out home safety checks, identifying emergency support contacts and completing action / support plansto ensure individuals can remain independently at home for as long as possible.

4.Support those referred to the service to access other appropriate services and resources, ensuring contact is made with the right individual / agency as agreed

5.Monitor the outcome of the support provided using a number of tools including health and wellbeing assessments, patient experience and satisfaction questionnaires.

6.Take referrals for the service under the direction of the Hospital Aftercare Coordinator, which may include ward assessments for discharge, telephone support calls or post discharge phone calls.

7.Assist the Hospital Aftercare Coordinator and work with volunteers toprovide warning telephone calls and/ or visits to help manage risks for older people during adverse weather conditions.

8.Assist in the production of newsletters andattend any meetings or promotional events that may be required, to promoteand raise the profile of the service.

9.Maintain clear and comprehensive records and help to support effective management systems and data, in accordance with organisational policy and external requirements.

10.Support the Data Assistant in maintaining accurate, up to date information and a directory of local support networks for older people in Salford, ensuring these are used to support the individual’s journey to independence and wellbeing.

11.Assist with the administrative functions for the team, including photocopying, collection of stationary, filing etc

12.Take responsibility for own and othershealth and safety and to undertake risk assessments of relevant activities.

1.Ensure compliance with all relevant service standards, policies and procedures of Age UK Salford

2.Developand maintain effective relationships with service users, their carers and all other key individuals and / or groups whilst demonstrating a positive commitment to providing good customer service at all times.

3.Communicate effectively by maintaining clear, concise and open communication at all times, ensuring that the Hospital Service Manager / Coordinator are kept fully informed and updated.

4.Attend and make positive contributions to regular team meetings and individual supervision.

5.Continuously review own performance and take responsibility for own learning and development by undertaking induction and on-going training as required.

6.Work with the other team members, external professionals and others to consistently deliver positive outcomes for those who use our services.

7.Demonstrate personal leadership by contributing toward continuous service improvement, adopting a positive approach to change and willingness to explore solutions to resolve the day to day challenges of delivering an effective service.

8.Ensure that the service offered is sensitive and responsivein meetingthe needs of the diverse communities in which they are delivered.

13.Undertake any other tasks, duties or projects as required which are relevant to the role.

PERSON SPECIFICATION – Community Support Worker

The following requirements will be assessed from a combination of information provided from the application form, panel interview process and references:

Education and Qualifications / Essential / Desirable
Degree or professional qualification
Relevant NVQ level 2 or equivalent and willingness to work toward level 3 / X / X
Skills, Knowledge and Experience / Essential / Desirable
3 years or more experience of working in the statutory or voluntary health &/or social care sectors.
Understanding of the health and wellbeing needs of older people and the impact on the health of carers.
Experience of undertaking risk / needs assessment and support planning processes.
Understanding of the principles of `safeguarding vulnerable adults’.
Experience of community work and knowledge of community support groups and activities available for older people in Salford.
Ability to work on own initiative and as part of a dynamic team.
Experience of working in partnership with other multi-disciplinary health &/or social care providers.
Excellent written, verbal and interpersonal communication skills.
Good analytical skills and ability to reflect on own practice.
Data management, word processing and IT skills.
Excellent organisation skills and the ability to maintain accurate and concise records
Knowledge and understanding of relevant social policy and legislation. / X
