INTRODUCTION
Since 1994 CMG has been supporting people with learning disabilities, Autism, physical disabilities, chronic and enduring mental health needs, sensory and communication impairments and associated complex needs including challenging behaviours.
This is the first evaluation report under Supporting People.
As part of the evaluation process, preparatory documentation was issued. To permit a more comprehensive evaluation additional supporting evidence was also requested. This report describes conclusions about the quality of the service based on previous monitoring information as well as the evaluation itself.
Outcomes achieved for service users have been considered under the following headings:
· Promoting Personal and Community Safety
· Promoting Independence and Control
· Promoting Economic Progress and Financial Control
· Promoting Health and Well Being.
CMG statement:
Our personalised services support people with learning disabilities including physical disabilities, mental health needs, Autism, Down’s Syndrome, complex health needs such as cystic fibrosis, long term neurological conditions such as Huntington’s disease, dementia and cerebral palsy and acquired brain injury (ABI). Our specialisms also include support for people with sensory and communication impairments, such as being deaf and/or blind and those who have limited or no verbal communication.
Our support always varies according to individual need in accommodation ranging from people’s own flats or family homes to supported living and residential services, including nursing. For many of the people we support with ABI, our supported living services represent their first step back to their communities from rehabilitation hospitals.
We understand that transition can be very daunting and we overcome this by providing an extremely person centred approach to the support and care that we provide, ensuring that transitions are tailored to meet individual needs and by providing individualised support packages which include 24 hour support where required. Our dedicated Placement Support Team provides additional transition help if required.
Everyone we support is assisted to develop a Person Centred Plan (PCP) that focuses on enabling each person to achieve their individual dreams and aspirations. These may include making friends, achieving increased independence and gaining education and/or employment.
Service users are supported by our teams to develop lifestyles of their choosing through our Person Centred Active Support (PCAS) approach. Our dedicated PCP and PCAS Co-ordinator is on hand to ensure that our teams and everyone they support continue to develop and achieve better outcomes.
Where alternative methods of communication are appropriate we support people to develop individualised communication strategies using our Total Communication approach. Alternative communication methods such as PECS or talk mats are used to help people to express their feelings and wishes and to make choices. In addition, our staff are trained to use a variety of communication support systems including Makaton and British Sign Language. We also employ a dedicated Communication Psychologist who makes sure all communication strategies are appropriate for each individual.
We also support people to develop individual Health Action Plans which help them to live healthy and safe lifestyles. Our Healthcare and Therapy team support the development of these plans and liaise with GPs and hospitals where necessary.
CMG AIMS:
Care Management Group Cymru will aim to provide progressive and holistic care (maintaining a person centred and therapeutic approach) to adults with learning disabilities, autism, psychological needs, associated mental health issues and some behaviours of a challenging nature. Support will be provided by an appropriately trained staff team to promote individual independence, life skills and other needs as identified.
Our prime aim will be to implement a carefully planned package of care, utilising a multi-disciplinary approach, targeted towards each individual’s needs and aspirations. One-to-one sessions will be implemented as identified to develop an individual’s daily living skills and life skills.
The promotion of choice, independence and empowerment will be an integral part of all Support planning allowing individuals to gain self-confidence, self-esteem, dignity and self-worth. Service users will be supported to develop appropriate practical skills and provided with emotional support allowing the individual the ability to make informed choices enabling them to take control and enhance their overall quality of life.
We actively support our service users’ to access their local community and develop proactive involvement within this, actively supporting individuals to become integrated and active members of the local community.
Care Management Group run 5 permanent supported accommodation schemes in the Vale of Glamorgan.
Burlington Street: The service is for women with learning difficulties. The service provides accommodation for women with learning difficulties who want to live in supported tenancies in the community. The service provides 24-hour support if necessary. Accommodation is provided in an ordinary house. Each service user has a separate room and shares other facilities with the other tenants in the property. People can use the accommodation and support services for as long as they feel they are suitable or until service users are able to move on.
Caradoc Avenue: The service is for adults with learning difficulties. The service provides accommodation and one to one support for adults with learning difficulties who want to live in supported tenancies in the community. Accommodation is provided in an ordinary house. Each service user has a separate room and shares other facilities with the other tenants in the property. People can use the accommodation and support services for as long as they feel they are suitable or until service users are able to move on.
Clos yr Harbwr: The service is for people over 18, with learning difficulties who need accommodation with specialist support.
The service provides 24 hour supported living for clients with learning difficulties who want to live in supported tenancies in the community. The service is provided in a purpose built bungalow. Each service user has their own room and shares the other facilities.
This is permanent supported housing and can be accessed for as long as the client feels they need to or until they are able to move on.
Cornwall Road: The service is for adults with learning difficulties. The service provides accommodation and one to one support for adults with learning difficulties who want to live in supported tenancies in the community. Accommodation is provided in an ordinary house. Each service user has a separate room and shares other facilities with the other tenants in the property. People can use the accommodation and support services for as long as they feel they are suitable or until they are able to move on.
Kingsland Crescent: The service is for adults with learning difficulties. The service provides accommodation for adults with learning difficulties who want to live in supported tenancies in the community. The service is provided in an ordinary three bed-roomed house. Each service user has a separate room and shares other facilities with the other tenants in the property. People can use the accommodation and support services for as long as they feel they are suitable or until they are able to move on.
The Contract Monitoring Officers representing the Vale of Glamorgan Council’s Supporting People Team were Hedd Wyn John and Dee Brunton. Burlington Street, Caradoc Avenue and Kingsland Crescent were visited as part of the review.

Do the services meet the needs of vulnerable people?
The services have clear up to date aims and objectives.
The needs mapping information in the Vale of Glamorgan reflect the demand for the services.
The services are line with the 2014 – 2017 local commissioning plan priorities.
Over the last year there have been a high number of service users supported in these services.
Is the scheme’s performance satisfactory, including good contract compliance, high utilisation/demand, adherence to eligibility criteria and acceptable evidence of support outcome?
Performance indicator returns evidence low voids. There are 2 voids out of 15 units.
Monitoring Returns, Annual Returns and Desktop Audit forms are satisfactory.
The provider has a good history of contract compliance.
The provider evidences positive support through outcome monitoring however the current outcomes scores cannot be validated at present. Changes in the outcomes collection process have been recommended in section 9 to remedy this.
Service users’ verbal feedback was positive however given the service users disabilities the nature of the feedback was limited.
During service visits SP Contract Monitoring Officers witnessed that service users have a good relationship with staff members.
This Objective has been partially met.
Do the schemes demonstrate good value for money?
The schemes compare well when benchmarked against the SP funding levels of other similar schemes in the area.
