Dear Reference Provider:

Your feedback is essential in the evaluation of the applicant before a decision can be made about invitations for an on-site interview. In order to be considered for the residency, candidates must have a completed application submitted through PhORCAS (the Pharmacy Online Residency Centralized Application Service) by the January 1st deadline. Your timely response will help to facilitate this process.

Applicants to our residency are required to submit recommendations by persons able to evaluate their qualifications for residency training. The Reference Writer template in PhORCAS changed in 2014; no program specific letter uploads or text boxes are allowed, however, you can enter a brief program-specific comment (up to 1000 characters). Please use that field to address any of the items below that are not otherwise covered in the standardized reference format. Submit your reference and recommendation through the PhORCAS system. See http://www.ashp.org/phorcas for further instructions and to access the Reference Writer Portal login screen.

You do not need to return this cover letter. Specific information about the residency they are applying for can be found at http:/www.augustahealth.org/pharmacy-residencies. Our PGY1 program has been accredited for over 40 years, originally as Medical College of Georgia and MCG Health, however the hospital legal name changed in 2016.

In the PhORCAS Standardized Reference grid comments, narrative comments and program-specific comment field, please provide as much of the following information as possible. Additional insights into the applicant’s qualifications are welcome.

1.  How long have you known the applicant and in what capacity (under narrative comments item 1)

2.  How well do you know the applicant (under narrative comments item 1)

3.  Academic ability, knowledge base, critical thinking skills and quality of work

4.  Motivation, initiative related to residency training, practice leadership and scholarship

5.  Key characteristics evaluated in the standardized reference grid (such as writing, communication skills, leadership, time management, assertiveness, emotional stability and maturity)

6.  Strengths and weaknesses of the applicant (under narrative comments items 2 and 3)

7.  Include under program-specific comment field:

Overall assessment of the applicant’s suitability as a resident at an academic medical center (including readiness for pharmacy practice) on the following scale:

Must interview;

Qualified / should interview; Qualified with some reservations / consider interview;

Do not interview/do not recommend; or Unable to recommend

Thank you in advance for your assistance in identifying the most qualified residency applicants to interview.
