2015Application Form

Eligibility:

  • Current graduating student entering a continuingeducationprogram at a college, university, business school, technical school, or planning to serve in the military.
  • The student must have continuously attended Allegheny-Clarion Valley High School during 9th – 12th grades
  • Parent/legal guardian is a resident of the Allegheny-Clarion Valley School District during the year scholarship funds are provided.
  • U.S. citizen and a resident of the Commonwealth of Pennsylvania
  • Student must not have a level II or level III misconduct on their discipline record
  • Preference will be given to students who are residents of the city of Parker and have in interest in the outdoors, music, or the performing arts.

Application Deadline:

  • All applications must be submitted by noon onFriday, April 24,2015, to the Senior High School Counselor, Mrs. Minnie Logue.
  • Two letters of recommendation (one from a teacher/employer and one from a community member (not a relative) must accompany the application.

 Incomplete applications or applications submitted after the deadline

will not be reviewed by the Selection Committee.

  • All applications must be typed. Answer questions with complete sentences and respond thoroughly to each question asked. Add extra pages if needed.
  • Each application must include a typed, one page (12-point font, double-spaced, with a 1 inch margin) essay about what it means to “Dream Big, Work Hard, and Act Responsibly” and how those values will be applied to your future.
