Community Impact Assessment Form / Submitted in January and June annually / Web-based form, accessible through the EvaluATOD website. Assesses impact of RPCs in communities that have received substantial assistance. / RPC submits information through web-based form; Wilder analyzes and reports
Community-member Technical Assistance Feedback Survey (including P&I and SPF-SIG grantees) / Next survey in 2015 / Has been done through an on-line survey and through key informant interviews. Will likely use an on-line survey in 2015. Assesses types of support received by community members, their satisfaction with the support and their suggestions for improvements. / RPC provides sample and contact information. Wilder does data collection, analysis, and reporting
Training
General training evaluations / Ongoing / Two generic surveys, available in paper and through Survey Monkey, to assess training satisfaction and impact. Wilder can customize the survey to the specific training. Generally administered in paper form to participants at the end of training. / RPC requests customization of survey and administers it to participants. Wilder customizes survey, does data entry, analysis, and reporting
Project Northland and Class Action Training of Teachers / Ongoing with reporting in the spring / These surveys assess satisfaction with and outcome of each training, and are generally administered in paper form at the end of the training. They are also available on Survey Monkey, if needed / RPC administers survey. Wilder does data entry, analysis, and reporting
Survey of training needs and preferences – Coalition trainings / Next survey in Autumn 2014 / Administered to coalitions in each region to assess training needs related to coalition building, infrastructure, and sustainability, as well as to prevention topics. Available in paper and via Survey Monkey. / RPC submits plan to Wilder for administering surveys and then administers them. Wilder provides data entry for paper forms, and data analysis and reporting for all data.
SAPST:Registration questions, daily surveys (includes overall survey), and follow-up survey / Ongoing / The registration and follow-up surveys are via Survey Monkey. The daily surveys are paper. The goal is to assess satisfaction with and outcomes of the SAPST training. / MPRC sends out registration survey. RPCs administerdaily surveys. Wilder administers follow-up survey and does all data entry, analysis, and reporting.
