Community Activities

Community work group and partners: For the 2016-2017 funding year, the PREP evaluation includes assessment of the activities of the community work group and community implementation partners. Mainly, these activities consist of community-wide events, the creation and maintenance of condom access points, and the creation of a referral network. The Community Workgroup will also be assessed to see how it functions and progresses towards its stated goals. There are three online forms in Qualtrics survey software that help us monitor these activities.

General Guidelines

The different online forms are explained in greater detail below, but please also keep these general things in mind:

Please fill out logs as soon as possible

Many of the activities will be completed in different time frames (some are at regular intervals, some are not). Whatever the planned interval, please complete the log as soon as possible so that we can stay up to date.

All logs allow multiple entries

If you have multiple activities of the same type (such as checking several condom access points), you may add another event or entry by indicating so on the last question.

When filling out forms,be as complete as possible

Depending on the activity, some fields may not be relevant. Those can be left blank (but do the best you can). If you have any questions, feel free to contact me at .

Activity and Community Events

The Community Work Group along with community partners are planning several events and activities for their communities that do not fit the category of evidence based programs: such as parent child communication events, workshops on healthy relationships, distribution of informational flyers, and information tables at other events such as health fairs. This activity form is designed to capture these activities and events.

This form records basic information, such as the sponsoring organization, name of activity and brief description. Due to the high degree of variation, not all questions on the form will be fit for all activities.

Condom Access Points

All Lead Coordinating Agencies agreed to periodically review their teen friendly condom access points, and deliver condoms. This form allows you to record that a condom access point is still active, meets criteria (such as being teen friendly, and having condoms accessible in a confidential manner), and also records the number of condoms delivered. Each Lead Coordinating Agency outlined different intervals for checking condom access points, so this form allows you to report the date range.

Referrals

Part of the PREP project for 2016-2017 is the creation of referral networks, so that youth can get services they need to remain healthy. The development of these networks (and subsequent evaluation) is ongoing, but this log will allow you to record the numbers of referrals made across different referral forms.

Community Work Group

In order to assess the progress of the Community Work Group (CWG), the evaluation team will be collecting and reviewing meeting minutes and tracking the frequency of meetings. The aim is to assess progress to the CWG’s stated goals.

In addition, CWG meeting will occasionally be attended by staff from the South Carolina Campaign to Prevent Teen Pregnancy, and we may also interview CWG members to get their perspective on the functioning of the workgroup.If the CWG sponsors any activities directly, they can be recorded in the event log described above.

Evidence Based Programs

Evidence Based Programs

The evidence based program evaluation consists of measures of fidelity to curriculum, attendance of participants,an entry survey and an exit survey.

General Information

Versions

Each evidence-based program has a different version of the fidelity log. They are program specific. There are also different versions of the entry survey (see below), depending on age of participants and location. The exit survey is the same for all participants.

Cohorts

All evaluation data collection for PREP (Entry and Exit, fidelity, and attendance) is separated by cohort. A cohort is one group of youth who have participated in an evidence-based program together, at the same time, in the same location, by the same facilitator. Separate materials need to be filled out for every cohort. Please do not combine cohorts.

Timeliness

Many of our tools are designed to be filled out upon the completion of eachlesson (such as attendance and fidelity). This ensures that information is accurate.

Please send all information electronically as soon as possible following the completion of a cohort.

Entry Survey

The entry survey should ideally be given to all youth at the same time and before the beginning of the program. If access to computers is limited, then youth may take it in smaller groups, but please make sure they are done as close to the same time as possible

Exit Survey

The exit survey should ideally be given to all youth at the same time and as close to the end of the program as possible (but not before the end of the program).If access to computers is limited, then youth may take it in smaller groups, but please make sure they are done as close to the same time as possible

Paper Based Option

We strongly encourage all sub-grantees to use our electronic tools because it reduces data entry burden, and allows for quicker turn around times for information. However, if electronic forms or surveys are not possible, we can provide you with paper versions and instructions.

Fidelity Measures

Fidelity measures for each cohort are combined in the same excel workbook, with tabs separating the sections. There are four parts total. Where applicable, information filled out in one section (such as lesson information) autofills to other relevant sections (such as attendance).

Tab 1: Program Information

This tab contains basic information such as facilitator name, location, and dates for the entry and exit surveys.

Tab 2: Lesson Information

This tab contains the information about each lesson taught, including the date and time the lesson was taught, the number of students present, and the length of the lesson in minutes and days (to capture lessons taught over multiple days).

Tab 3: Fidelity

In this tab, facilitators report whether they completed the activities in a lesson, and whether they made any changes.

Tab 4: Attendance

In this tab, facilitators record, for each student whether they were present or absent during a lesson (1=present, 0=absent). Please delete any student names before sending.

Surveys all surveys contain pre and post tests about STI knowledge, and self efficacy to refuse sex or use a condom. There are different version of the entry survey based on age and location.

Entry Survey

There are three versions of the entry survey, depending on location:

In school, older youth

Link:

This version of the survey includes a question on sexual orientation, as well as some questions on past sexual behavior and pregnancies. This survey is for grade 9 and older.

In school, younger youth

Link:

This survey omits the sensitive questions on sexual orientation and sexual behavior. The rest of the format is the same as the older youth survey in school. This survey is for grade 8 and below.

Community survey

Link:

There is one survey for programs conducted in the community, rather than in school during the school day. In this survey, sensitive questions are displayed if the youth reports that they are 14 or older, and they are hidden if the youth indicates they are 13 or younger. This survey is designed for venues where there are students of mixed ages in a cohort. If you are concerned that younger youth may see sensitive questions, then you can also use the younger youth in school survey, but please notify the evaluation team.
