I am sorry to hear that you are unwell and unable to attend work. Although I do understand the reason for your continued absence, my understanding is that your most recent Statement of Fitness for Work expired on <date> and I was therefore expecting you to return to work on <date>. I am concerned that I have not received a subsequent Statement of Fitness for Work to cover your absence and you have not been in touch to let me know when you intend to return to work.

As you are probably aware, the sickness regulations that are part of your employment terms and conditions require that you provide Statements of Fitness for Work to cover continuous absence of over seven days.

You should be aware that, if I have not heard from you by <date - seven days after the date of this letter, I will seek advice from HR Operations regarding your continued absence. A possible outcome may be that I consider your absence to be unauthorised and that will have an impact on your pay.

Please contact me urgently by telephone, email or in person to let me know the reason for your continued absence and to indicate your intentions regarding returning to work. My contact details are at the top of this letter.
