Personal Growth Plan (PGP) Plan

This template is to assist you in breaking down the steps of the PGP to make it more manageable. Please check the most updated rubric for requirements to score at the Distinguished/Proficient levels as these have changed recently. This template is to assist but does not replace checking your work against the rubric.

Rationale for the PGP Goal

  1. What does the data show? (Rubric 1)
  • List and describe the data source you used (Proficient)
  • List and describe the multiple data sources (Rubric #1), both quantitative and qualitative, used (Rubric #2)
  • What were the results of the data? (Recommended to use whole numbers)
  • In what areas did students struggle?
  1. Cite and describe the research that provides evidence based practice to address the need identified through the data.

Remember: This connects to your individual professional development.

Within school (Proficient-Rubric #3):

Beyond the school (Distinguished-Rubric #3): PD360, Professional Learning Library/Gale resource website, BEST site, FCRR, District Resource Teachers, etc

  1. Self reflect on your strengths and needs related to the data
  • What are your instructional strengths?
  • What are your growth areas based on the data?
  • Are students involved in data analysis and/or instructional delivery improvement efforts?

Professional Growth Plan Goal (Rubric #7)

Write one to two sentences that describe what *new* professional practices you will learn and implement.

“I will ______that will lead to enhanced student performance on ______. As a part of this effort I will ______.”

Ex. I will develop and utilize individual and group formative assessment practices that lead to enhanced student performance on the Florida Vocal Association Music Performance Assessment. As a part of this effort, I will create meaningful ways to involve students in both assessment development and progress monitoring.

What new skill are you going to learn and do that will lead to enhanced student performance?

*In order to be a “stretch” goal, this needs to be something new that is not part of your current classroom practice (Distinguished Rubric #7)
