NOTICE

Applications are invited from External Candidates who have passed final BDS Examination on or before 31.10.2014 and wish to do internship at Maulana Azad Institute of Dental Sciences, New Delhi against the vacant seat. Application along with a fee of Rs. 1000/- (Non-Refundable) in cash and following documents may be submitted to Registrar, Maulana Azad Institute of Dental Sciences, New Delhi as per schedule below:-

LIST OF DOCUMENTS TO BE ATTACHED

1. Reasons by the candidate to do internship at Maulana Azad Institute of Dental Sciences.

1. No Objection Certificate from his/her passing out college stating that the college was not derecognized by Dental Council of India during the course of training from admission to his/her passing out and the college is duly recognized by Dental Council of India subject to selection at MAIDS.

1. Permission of the University to which the parent College is affiliated to do internship at MAIDS subject to selection.

1. Permission of the Dental Council of India to do internship at MAIDS subject to selection and certificate from Dental Council of India that Dental College/Institution where the student at present studying has been recognized by the Dental Council of India from the year the student was admitted in this Institution to the year of his/her passing out.

1. Character and conduct Certificate of the student from the parent College/Institution.

1. Self Attested photocopy of mark sheets of all the BDS examination (for each part/year) passed.

1. Attempt Certificate of passing BDS (each part/year).

1. Documentary evidence, distinction/academic Medal, if any awarded for any subject in University Examination.

1. An undertaking that the student is prepared to do internship without any pay, stipend or honorariumand willdeposit Rs. 2, 00,000/- (Rs.Two Lakh only) as Internship Fee if got selected, in the form of Demand Draft in favour of “Director-Principal’’, MAIDS New Delhi, payable at New Delhi.

1. A deposit in the form of DD of Rs.10,000/- (Rs. Ten Thousand only) in favour of “Director-Principal”, MAIDS, New Delhi which is refundable only in case the selection is not done. In case of selection, it will be adjusted within the total fee of Rs.2,00,000/- (Rs. Two Lakh only). It will not be refunded in any case, if the candidate does not join after his/ her selection.

1. Undertaking that he/she will follow the Internship programme of Maulana Azad Institute of Dental Sciences, New Delhi.

1. Undertaking that he/she would maintain good conduct, discipline and decorum of the Institution and the authorities of Maulana Azad Institute of Dental Sciences, New Delhi have the right to discontinue his/her internship at any time for his/her misconduct, indiscipline and unsatisfactory work.

1. Two passport size photographs, one affixed on the application form.

NOTE

1.Individual who have earlier submitted their request for internship have to apply afresh along-with the required documents as above with prescribed proforma.

2.Internship Fee of Rs. 2,00,000/- (Rupees Two Lakh Only) to be paid by the selected candidates before the start of training. The fee is non-refundable after the training is started.

3.Application without requisite documents as per this notice will be summarily rejected and no extension will be granted for submission of any documents. Application fee of Rs. 1000/- will also stand for feited.

4.No application will be entertained after 20.11.2014 at 4.00 PM. The Institute will not be responsible for any delay on account of postal delivery or delivery through any other agency.
