TITLE OF PROPOSED STUDY

BACKGROUND
State the problem or question the research is designed to address and indicate to which specific topic in the funding announcement this research responds. Describe why this is of interest to patients,caregivers, and/or decision makers. Include a brief study rationale and how it ties to the conceptual/theoretical framework guiding your proposed study.

OBJECTIVES

Briefly describe the specific aims of the study, including specific research question(s) and long-term objectives.

METHODS

Provide a concise description of the study design (including theoretical or conceptual framework), study population (including inclusion/exclusion criteria), sample size, and analytic methods that will be employed.Clearly describe the proposed comparators and provide evidence of their previously demonstrated efficacy.

PATIENT OUTCOMES (PROJECTED)
Specify the study outcomes, and state briefly why they are important to patients.

PATIENT AND STAKEHOLDER ENGAGEMENT

Provide a clear and concise justification of how patients, family members, caregivers, and the organizations thatrepresent them, as well as any other relevant stakeholders, will be engaged in study activities. Describe how patients and stakeholders will be involved in planning the study, conducting the study, and disseminating study results.

ANTICIPATED IMPACT

Briefly describe your dissemination plan, including how your study findings will be used in healthcare decision making and the anticipated impact of your research findings. Please describe how this anticipated impact will help to advance PCORI’s mission.
